European Society for Medical Oncology (ESMO) position paper on supportive and palliative care

2018

Oncology has come a long way in addressing patients' quality of life, together with developing surgical, radio-oncological and medical anticancer therapies. However, the multiple and varying needs of patients are still not beingmet adequately as part of routine cancer care. Supportive and palliative care interventions should be integrated, dynamic, personalised and based on best evidence. They should start at the time of diagnosis and continue through to end-of-life or survivorship. ESMO is committed to excellence in all aspects of oncological care during the continuum of the cancer experience. Following the 2003 ESMO stand on supportive and palliative care (Cherny N, Catane R, Kosmidis P. …

Pharmacogenetics of treatments for pancreatic cancer

2019

Introduction: Despite clinical efforts, pancreatic ductal adenocarcinoma (PDAC) has a dismal prognosis. The scarcity of effective therapies can be reflected by the lack of reliable biomarkers to adapt anticancer drugs prescription to tumors’ and patients’ features. Areas covered: Pharmacogenetics should provide the way to select patients who may benefit from a specific therapy that best matches individual and tumor genetic profile, but it has not yet led to gains in outcome. This review describes PDAC pharmacogenetics findings, critically reappraising studies on polymorphisms and -omics profiles correlated to response to gemcitabine, FOLFIRINOX, and nab-paclitaxel combinations, as well as l…

Rituximab in primary Sjögren's syndrome: a ten-year journey

2014

Primary Sjögren's syndrome (pSS) is an autoimmune disorder affecting exocrine glands and characterized in most cases by a rather mild clinical picture. However, a subgroup of pSS patients experience systemic extraglandular involvement leading to a worsening of disease prognosis. Current therapeutic options for the treatment of pSS are mainly empirical, often translated by other autoimmune diseases, and recent systematic reviews have highlighted the lack of evidence-based recommendations for most of the drugs commonly employed in the spectrum of extraglandular involvement. Because of the well-established role of B-lymphocytes in the pathogenesis of pSS, a B-cell targeting therapy may repres…

Virtual Clinical and Precision Medicine Tumor Boards-Cloud-Based Platform-Mediated Implementation of Multidisciplinary Reviews Among Oncology Centers…

2021

Background Multidisciplinary tumor boards play a pivotal role in the patient-centered clinical management and in the decision-making process to provide best evidence-based, diagnostic, and therapeutic care to patients with cancer. Among the barriers to achieve an efficient multidisciplinary tumor board, lack of time and geographical distance play a major role. Therefore, the elaboration of an efficient virtual multidisciplinary tumor board (VMTB) is a key point to successfully obtain an oncology team and implement a network among health professionals and institutions. This need is stronger than ever during the COVID-19 pandemic. Objective This paper presents a research protocol for an obse…

“TEACHING REAL NUMBERS IN THE HIGH SCHOOL: AN ONTO-SEMIOTIC APPROACH TO THE INVESTIGATION AND EVALUATION OF THE TEACHERS' DECLARED CHOICES”

The thesis addresses the topics of investigating teachers' declared choices of practices concerning real numbers and the continuum in the high school in Italy, evaluating their didactical suitability and the impact of a deep reflexion about some historical and didactical issues on the teachers' decision-making process. Our research hypothesis was that teachers' choices of teaching sequences concerning real numbers, with particular attention to the representations of real numbers, could be very relevant in order to interpret some of the well-known students' difficulties. After a pilot study in form of a teaching experiment and a literature review concerning students' and teachers' difficulti…

Ontological information system for the selection of technologies for the treatment and disposal of organic waste: engineering and educational aspects

2021

The article proves a problem of information processing in the field of the structuring of information and decision-making in environmental engineering, including education in this field. This article presents the ontology-information model that was used to systematize waste treatment equipment. The article presents the use of an ontological algorithm “Polyhedron” for engineers and teachers. The algorithm consists of integrating ontological solutions from abstract to specific. The proposed model consists of a general ontology of “waste treatment technologies selection”, “waste treatment equipment selection” and “Ontology for the analysis of scientific publications”. Users of the system can i…

Robotic-assisted right colectomy. Official expert recommendations delivered under the aegis of the French Association of Surgery (AFC)

2022

: Twenty-seven experts under the aegis of the French Association of Surgery (AFC) offer this reference system with formalized recommendations concerning the performance of right colectomy by robotic approach (RRC). For RRC, experts suggest patient installation in the so-called "classic" or "suprapubic" setup. For patients undergoing right colectomy for a benign pathology or cancer, RRC provides no significant benefit in terms of intra-operative blood loss, intra-operative complications or conversion rate to laparotomy compared to laparoscopy. At the same time, RRC is associated with significantly longer operating times. Data from the literature are insufficient to define whether the robot f…

The neurochemistry of autism

2020

Autism spectrum disorder (ASD) refers to complex neurobehavioral and neurodevelopmental conditions characterized by impaired social interaction and communication, restricted and repetitive patterns of behavior or interests, and altered sensory processing. Environmental, immunological, genetic, and epigenetic factors are implicated in the pathophysiology of autism and provoke the occurrence of neuroanatomical and neurochemical events relatively early in the development of the central nervous system. Many neurochemical pathways are involved in determining ASD; however, how these complex networks interact and cause the onset of the core symptoms of autism remains unclear. Further studies on ne…

Seismic shakedown design of frames based on a probabilistic approach

2014

The present study concerns the optimal design of elastic perfectly plastic structures subjected to a combination of fixed and seismic loads. In particular, plane frames are considered and suitable measures of the beam element cross sections are chosen as design variables. The optimal design is required to behave in a purely elastic manner when subjected just to the fixed load and to have the capability to eventually shakedown when simultaneously subjected to fixed and seismic loads. Due to the natural uncertainness related to the definition of the seismic load history, a new probabilistic approach is proposed, consisting into two subsequent search steps. At first a suitably chosen large num…

A gradient-based decomposition approach to optimize pressure path and counterpunch action in Y-shaped tube hydroforming operations

2008

International audience; In tube hydroforming, the concurrent actions of pressurized fluid and mechanical feeding allows obtaining tube shapes characterized by complex geometries such as different diameters sections and/or bulged zones. Main process parameters are material feeding history (i.e., the punches velocity history), internal pressure path during the process, and (in T- or Y-shaped tube hydroforming) counterpunch action. What is crucial, in such processes, is the proper design of operative parameters aimed to avoid defects (for instance underfilling or ductile fractures). Actually, the design of tube hydroforming operations is mainly aimed to prevent bursting or buckling occurrence …
