Extubation strategies in neuro-intensive care unit patients and associations with outcomes
2020
Background Prolonged invasive ventilation is common in patients with severe brain injury. Information on optimal management of extubation and on the use of tracheostomy in these patients is scarce. International guidelines regarding the ventilator liberation and tracheostomy are currently lacking. Methods The aim of 'Extubation strategies in Neuro-Intensive care unit patients and associations with Outcomes' (ENIO) study is to describe current management of weaning from invasive ventilation, focusing on decisions on timing of tracheal extubation and tracheostomy in intensive care unit (ICU) patients with brain injury. We conducted a prospective, international, multi-centre observational stud…
PODOLOGICAL ASPECTS OF FOOT CARE IN SOCIAL CARE CENTERS
2018
Investigate the necessity for therapeutic foot care and the involvement of a podologist in an interdisciplinary health care team in social care institutions in Latvia. So far, no study has been conducted on the need for therapeutic pediatric care for Social Care Center (SCC) clients in Latvia; it is known that in individual SCC’s where some podologists work part-time, another podologist is invited in specific cases, or clients are taken to a podologist at a medical institution. The study was conducted in 15 Latvian Social Care Centers (SCC), surveying 750 customer feet. People who, for various reasons, are no longer capable of foot care, is a highly necessary specialist who will do it profe…
COVID-19 Vaccine and Death: Causality Algorithm According to the WHO Eligibility Diagnosis
2021
The current challenge worldwide is the administration of anti-severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) vaccines. Even if rarely, severe vascular adverse reactions temporally related to vaccine administration have induced diffidence in the population at large. In particular, researchers worldwide are focusing on the so-called “thrombosis and thrombocytopenia after COVID-19 vaccination”. This study aims to establish a practical workflow to define the relationship between adverse events following immunization (AEFI) and COVID-19 vaccination, following the basic framework of the World Health Organization (WHO). Post-mortem investigation plays a pivotal role to support this c…
The FRAILMar Study Protocol: Frailty in Patients With Advanced Chronic Kidney Disease Awaiting Kidney Transplantation. A Randomized Clinical Trial of…
2021
Introduction: Frailty is very frequent among patients with chronic kidney disease (CKD) who are awaiting deceased donor kidney transplantation (KT), and transplant outcomes are worsened in those frail recipients. Frailty and poor fitness powerfully predict mortality, kidney graft survival, and healthcare utilization after KT. Intervention is essential to improve survival and quality of life for frail CKD patients, regardless of their age. Studies of post-transplant physical therapy intervention have been met with limited success, in large part due to high dropout rates. A pre-transplant clinical framework for multimodal prehabilitation interventions including physical therapy, nutritional m…
Chemotherapy for advanced gastric cancer
2017
Background Gastric cancer is the fifth most common cancer worldwide. In "Western" countries, most people are either diagnosed at an advanced stage, or develop a relapse after surgery with curative intent. In people with advanced disease, significant benefits from targeted therapies are currently limited to HER-2 positive disease treated with trastuzumab, in combination with chemotherapy, in first-line. In second-line, ramucirumab, alone or in combination with paclitaxel, demonstrated significant survival benefits. Thus, systemic chemotherapy remains the mainstay of treatment for advanced gastric cancer. Uncertainty remains regarding the choice of the regimen. Objectives To assess the effica…
Medico legal procedures related to sexual assault: a 10-year retrospective experience of a Daphne protocol application
2018
Background: Sexual assault is a worldwide problem that has not yet been sufficiently acknowledged as confirmed by the literature. Giacconeâ University Hospital of Palermo in 2006. Aim: data analysis emerging from 10 years experience of Daphne protocol utilization for the management of sexual assault victims. Methods:…
