Evolving European guidance on the medical management of neovascular age related macular degeneration

2006

BACKGROUND: Until recently, only two options were available for the treatment of choroidal neovascularisation (CNV) associated with age related macular degeneration (AMD)-thermal laser photocoagulation and photodynamic therapy with verteporfin (PDT-V). However, new treatments for CNV are in development, and data from phase III clinical trials of some of these pharmacological interventions are now available. In light of these new data, expert guidance is required to enable retina specialists with expertise in the management of AMD to select and use the most appropriate therapies for the treatment of neovascular AMD. METHODS: Consensus from a round table of European retina specialists was obt…

Transpupillary thermotherapy (TTT) for age-related macular degeneration (AMD) – Is it time to say good-bye?

2010

Abstract Purpose To evaluate the effectiveness of transpupillary thermotherapy (TTT) for the closure of classic and occult subfoveal choroidal neovascularization (CNV) and to discuss whether TTT is a useful treatment modality for age-related macular degeneration (AMD). Materials and methods The present paper is a retrospective review of the clinical results from a patient group which was treated with TTT in the University Eye Clinic in Mainz. From January 2000 to December 2002, a total of 20 eyes of 20 patients with subfoveal CNV (due to AMD in 16 eyes, due to pathologic myopia in 4 eyes) received TTT. The procedure was delivered using an infrared diode laser at 810 nm, equipped with a modi…

Classic Choroidal Neovascularization

2009

Since Donald Gass’s pioneering work in 1967, which demonstrated the role of choroidal neovascularization (CNV) in wet (exudative) AMD, visible (classic) CNV, well demarcated and well-defined on fluorescein angiography, has become the most characteristic form of AMD.

Medical therapy or revascularization for patients with chronic total occlusion? A dilemma almost solved

2020

Chronic total occlusion (CTO) of the coronary arteries are relatively common, observed in approximatively 15-25% of patients with coronary artery disease undergoing coronary angiography. The right coronary artery represents the most common CTO vessel, which represents about half of the CTO cases. The CTO prevalence is much higher ( 90%) among patients with prior coronary artery bypass graft (CABG), while a CTO is found in only one tenth of patients referred for ST-elevation myocardial infarction (STEMI)

Revacept, an Inhibitor of Platelet Adhesion in Symptomatic Carotid Artery Stenosis: Design and Rationale of a Randomized Phase II Clinical Trial

2020

AbstractPatients with stroke or transient ischemic attacks (TIAs) and internal carotid artery stenosis harbor an increased risk of recurrent stroke especially within 2 weeks after the first event. In addition, the revascularization procedure itself (carotid endarterectomy [CEA] or carotid artery stenting [CAS]) is associated with both clinically apparent and silent brain infarctions, mainly caused by the embolic nature of the ruptured carotid plaque. The glycoprotein VI (GPVI) fusion protein Revacept is a highly specific antithrombotic drug without direct inhibition of systemic platelet function that might reduce periprocedural distal embolization from the vulnerable ruptured plaque located…

Determination of myocardial perfusion and coronary flow reserve by densitometric measurements: a selective classification of regions of interest

2002

To evaluate the functional effect of a coronary artery stenosis on the dependent myocardium, digitized coronary angiograms (DCAs) were recorded. The DCAs, acquired during heart catheterization, provide information about myocardial perfusion and coronary flow reserve of areas supplied by this coronary artery. For each analysis the myocardium is separated into different regions of interest (ROIs); 16 ROIs in the distribution of the LCA and 8 ROIs for the RCA. In addition to that, the ROIs of the diseased vessels are divided into ROIs which are proximal, within and distal to the stenosis. Using a semiautomatic procedure, densograms are constructed from which the parameter rise time is calculat…

Automated classification of neurodegenerative parkinsonian syndromes using multimodal magnetic resonance imaging in a clinical setting

2020

ABSTRACTBackgroundSeveral studies have shown that machine learning algorithms using MRI data can accurately discriminate parkinsonian syndromes. Validation under clinical conditions is missing.ObjectivesTo evaluate the accuracy for the categorization of parkinsonian syndromes of a machine learning algorithm trained with a research cohort and tested on an independent clinical replication cohort.Methods361 subjects, including 94 healthy controls, 139 patients with PD, 60 with PSP with Richardson’s syndrome, 41 with MSA of the parkinsonian variant (MSA-P) and 27 with MSA of the cerebellar variant (MSA-P), were recruited. They were divided into a training cohort (n=179) scanned in a research en…

Parametric assessment of myocardial perfusion by densitometric evaluation of digital subtraction coronary angiograms: a comparison with tomographic T…

2002

Densitometric results about local myocardial perfusion are compared with those derived from a quantitative analysis of TL-201 single photon emission computer tomograms (TL-201 SPECT). Ten patients underwent conventional TL-201 SPECT at rest and after maximum bicycle stress and dipyridamole infusion the day prior to the investigation. SPECT was applied to provide improved spatial resolution for the detection of perfusion abnormalities. During the catheterization procedure four digital subtraction coronary angiograms were performed, two of the left and two of the right coronary artery both before and after pharmacologically induced hyperemia by intracoronary papaverine. The densitometric para…

Intravascular Ultrasound-based Imaging Modalities for Tissue Characterisation

2014

Atherosclerosis is the leading cause of cardiovascular mortality and morbidity in the developed world. Intravascular ultrasound (IVUS) is a widely used imaging modality providing complementary diagnostic information to angiography regarding the vessel wall of the coronary arteries. IVUS has been used for assessment of ambiguous angiographic lesions, evaluation of new interventional devices and in atherosclerosis progression-regression trials. However, the standard gray-scale IVUS has limited value for the accurate identification of specific plaque components. This limitation has been partially over- come by introduction of new IVUS-based imaging methods such as: virtual histology IVUS, iMAP…
