Endoluminal ultrasonic examination of sessile polyps and early carcinomas of the rectum

The results of conventional endosonographic techniques in the assessment of early carcinomas and sessile polyps of the rectum have been unsatisfactory. We therefore developed a new technique in which the rectal cavity is filled directly with water. Using this technique, the anatomy of small rectal tumors is preserved and the layers of the rectal wall are easier to differentiate, especially with a 10-MHz scanner. The clinical results in 66 patients demonstrate that this new technique is very accurate in the preoperative staging of adenomas and T1-carcinomas of the rectum.

Predictive value of IL-6 and IL-10 serum levels in anastomotic leakage in elderly patients undergoing surgery for colorectal cancer

Methods The study group is yet represented by 33 patients: 17 males and 16 females (median age 76, range 70–89). The location of cancer was respectively: 5 (15, 15%) in the rectum, 15 (45, 45%) in the left colon, 11 (33, 33%) in the right colon, 1 (3, 3%) in the transverse colon, 1 (3, 3%) synchronous in the caecum and the sigmoid colon. The associated diseases observed were: chronic obstructive pulmonary disease, hypertension, diabetes mellitus, ischemic heart disease arterial vasculopathy.

With disease processes that involve the large bowel but leave the rectum free (Crohn’s disease, diverticulitis, neoplasms), a surgical procedure used for their treatment will result in an anastomosis that may be seen during rectoscopy. During the postoperative follow-up period, careful attention should be given to any recurrence, which in cases of Crohn’s disease is seen very early on rectoscopy.

Pelvic Autonomic Nerve Preservation during Total Mesorectal Excision (TME) from Werner Kneist

According to the German guideline for “Colorectal Cancer,” total mesorectal excision (TME) removes the cancer located at the central and lower thirds of the rectum and the pelvic floor while preserving the superior hypogastric plexus (SHP), the hypogastric nerves, and the inferior hypogastric plexus (IHP) (recommendation level A, level of evidence 1b, strong consensus). Proctitis and constipation: A symptom of food allergy?

Abstract Allergic proctitis can occur in infants and children. It is expressed by feces streaked with blood without other apparent signs. More than 50% of these patients are being breast-fed. Recently, it has been found that a particular symptom of allergic proctitis is constipation, and it is a major cause of consultations in pediatric gastroenterology departments. Prognostic implications of circumferential location of distal rectal cancer

Aim  This study evaluated the prognostic importance of circumferential tumour position of mid and low rectal cancers. Method  All uT2, uT3 and uT4 tumours of the middle and lower rectum that underwent total mesorectal excision (TME) with curative intent between 1996 and 2006 were included. The predominant circumferential tumour position (anterior, posterior or circumferential) was defined on preoperative endorectal ultrasound examination (ERUS). Selection criteria between the abdominal or perineal approach or between rectopexy and resection rectopexy are not yet proven.
