Simplifying atrial fibrillation ablation: how far can we go?

The procedural end point of electrical isolation of the pulmonary veins (PVs) for the treatment of atrial fibrillation (AF) has been debated extensively. After identifying the PVs as the dominant arrhythmogenic site of paroxysmal AF, 2 fundamentally different approaches to the interventional treatment of AF were introduced. Combined medical and mechanical recanalization in acute myocardial infarction

A technique of combined medical and mechanical recanalization was employed in 96 patients with acute transmural myocardial infarction. The mean time between onset of symptoms and admission to hospital was 170 +/- 65 min (X +/- SD). After 10 +/- 16 min, 250,000 U streptokinase was administered intravenously for 20 min. Intracoronary thrombolysis was commenced within 38 +/- 14 min. First coronary angiograms demonstrated reperfusion, an open vessel in 25/96 patients (26%). In 15/71 patients (21%) reperfusion occurred during thrombolysis therapy, before mechanical recanalization could be performed. Nerves projecting from the intrinsic cardiac ganglia of the pulmonary veins modulate sinoatrial node pacemaker function

Rationale: Autonomic nerves from sinoatrial node (SAN) ganglia are known to regulate SAN function. However, it is unclear whether remote pulmonary vein ganglia (PVG) also modulate SAN pacemaker rhythm. Objective: To investigate whether in the mouse heart PVG modulate SAN function. Ischemic stroke risk during long-term follow up in patients with successful catheter ablation for atrial fibrillation in Korea

The interruption of oral anticoagulation therapy (OAC) after CA of atrial fibrillation (AF) is controversial. The purpose of this study was to evaluate the relationship between successful long-term outcomes of catheter resection and SR maintenance and ischemic stroke risk in Korea. We studied 1,548 consecutive patients who were followed up for more than 2 years after CA of AF. We investigated the incidence of ischemic stroke during long-term follow-up. Three dimensional reconstruction to visualize atrial fibrillation activation patterns on curved atrial geometry

BackgroundThe rotational activation created by spiral waves may be a mechanism for atrial fibrillation (AF), yet it is unclear how activation patterns obtained from endocardial baskets are influenced by the 3D geometric curvature of the atrium or ‘unfolding’ into 2D maps. We develop algorithms that can visualize spiral waves and their tip locations on curved atrial geometries. The present study was conducted to test whether the well-known circadian alterations in physiological and metabolical parameters of the hypothalamic suprachiasmatic nucleus (SCN) are accompanied by day-night differences in the number of vesicles in intrinsic synapses of the nucleus. The circadian timing system is a neural network consisting of the hypothalamic suprachiasmatic nucleus, aided by the retina, other hypothalamic nuclei, autonomic regions of the spinal cord, sympathetic ganglia and the pineal gland. Spiral wave induced numerically using electrical stimulation and comparison with experimental results.

Experiments in vitro on a Microelectrode Array (MEA) platform show that electrical stimulation can provoke the generation of spiral waves in cardiac tissue. Nevertheless, the conditions leading to this artificial fibrillation state remain unclear. In order to have a better understanding of this phenomenon, a numerical simulation study has been conducted. The results obtained with a two-dimensional FitzHugh-Nagumo model proved that it is possible to create spiral waves by adding a stimulation current under certain conditions, which are made explicit.
