Derivation and validation of a novel bleeding risk score for elderly patients with venous thromboembolism on extended anticoagulation

2017

SummaryExisting clinical scores do not perform well in predicting bleeding in elderly patients with acute venous thromboembolism (VTE). We sought to derive an easy-to-use clinical score to help physicians identify elderly patients with VTE who are at high-risk of bleeding during extended anticoagulation (>3 months). Our derivation sample included 743 patients aged ≥65 years with VTE who were enrolled in a prospective multicenter cohort study. All patients received extended anticoagulation with vitamin K antagonists. We derived our score using competing risk regression, with the time to a first major bleeding up to 36 months of extended anticoagulation as the outcome, and 17 candidate var…

Diagnostic Performance of an Artificial Intelligence System in Breast Ultrasound.

2021

Objectives We study the performance of an artificial intelligence (AI) program designed to assist radiologists in the diagnosis of breast cancer, relative to measures obtained from conventional readings by radiologists. Methods A total of 10 radiologists read a curated, anonymized group of 299 breast ultrasound images that contained at least one suspicious lesion and for which a final diagnosis was independently determined. Separately, the AI program was initialized by a lead radiologist and the computed results compared against those of the radiologists. Results The AI program's diagnoses of breast lesions had concordance with the 10 radiologists' readings across a number of BI-RADS descri…

Exploring why global health needs are unmet by research efforts: the potential influences of geography, industry and publication incentives

2020

[Background]: It has been well established that research is not addressing health needs in a balanced way — much more research is conducted on diseases with more burden in high-income countries than on those with more burden in lower-income countries. In this study, we explore whether these imbalances persist and inquire about the possible influence of three factors, namely geography, industry and publication incentives.

Patient safety culture in Italian out-of-hours primary care service: a national cross-sectional survey study.

2020

BackgroundOut-of-hours (OOH) services in Italy provide >10 million consultations every year. To the authors' knowledge, no data on patient safety culture (PSC) have been reported.AimTo assess PSC in the Italian OOH setting.Design & settingNational cross-sectional survey using the Safety Attitudes Questionnaire — Ambulatory Version (SAQ-AV).MethodThe SAQ-AV was translated into Italian and distributed in a convenience sample of OOH doctors in 2015. Answers were collected anonymously by Qualtrics. Stata (version 14) was used to estimate Cronbach’s alpha, perform exploratory and confirmatory factor analysis, correlate items to doctors’ characteristics, and to do item descriptive analysis…

Exploring User-Related Drivers of the Early Acceptance of Certified Digital Stress Prevention Programs in Germany.

2020

Electronic mental health services represent innovative instruments to increase the dissemination of stress programs in primary prevention. However, little is known about facilitators of their uptake. This study aimed to explore determinants of the acceptance of centrally certified digital stress coping programs and preferences for service delivery modes among adult members of German statutory health insurances. Participants completed a multi-construct 45-item questionnaire covering acceptance of digital stress prevention (behavioral use intention) and potential predictors we assessed using hierarchical regression analysis—(1) socio-demographic variables and time spent online, (2) openness …

Foster Care: A Fragile Reality Needing Social Attention, and Economic Investments.

2016

The combination of psychosocial, medical, and educational care usually leads to immediate and long-term extra cost, which may pay out only decades later. This social and economic investment may be regarded by economists as being hardly affordable, especially during recession periods. However, we strongly believe that there are alternatives allowing adequate care even in times of economic turmoil. The aim of this report is to raise the awareness of the pediatric community and the public opinion toward the importance of investing in health programs offering adequate protection, prevention, and provision of care for orphan, abandoned, maltreated, and neglected children.

P-515 - CGI Rating to predict antidepressant treatment under naturalistic conditions

2012

Introduction Recent studies have shown that psychopathological rating by the Hamilton Depression Rating scale (HAMD) is well established and highly predictive for later response. In this study we aimed to find out if Clinical Global Impression (CGI) scale is suitable to guide antidepressive treatment under naturalistic conditions. Methods Inpatients with a major depressive disorder and treatment with citalopram were included and rated using in parallel the HAMD scale and the CGI scale weekly at baseline to day 35. According to CGI the sample has been divided in “CGI improver” (CGI = 2–4) and “CGI non-improver” (CGI = 5–6). Response was defined as HAMD sum score reduction by at least 50%. Re…

Psychiatric and behavioural symptoms and the use of psychotropic medication in Special Care Units and Regular Units in Norwegian nursing homes

2008

Background:  In a number of countries Special Care Units (SCUs) have been established to meet the particular needs of patients with dementia. The criteria for SCUs are poorly defined and often not met. Aim:  To assess the frequency distributions of dementia, psychiatric and behavioural symptoms and the use of psychotropic medication in SCUs and Regular Units (RUs) across different regions. Methods and material:  By means of a structured interview, comprising the Neuropsychiatric Inventory, the Clinical Dementia Rating Scale and Lawton’s Physical Self-maintenance scale, a representative sample of 1163 nonselected nursing home patients was assessed. In addition, information was collected from…
