Real-World Management of Myelofibrosis with Ruxolitinib: Initial Analysis of an Italian Observational Study (ROMEI)

2018

Abstract Introduction ROMEI (CINC424AIT04 Ruxolitinib Observational study in Myelofibrosis treated patiEnts in Italy) is a prospective observational study that aims to bridge the knowledge gap between the clinical experience of registration trials and routine patient management by following roughly 200 myelofibrosis (MF) patients (pts) treated with ruxolitinib in everyday clinical practice. Enrollment began in April 2017 and ended in May 2018. Methods The primary endpoint is to evaluate changes in symptoms and quality of life during treatment with ruxolitinib through the Myeloproliferative Neoplasm 10 (MPN-10) disease-specific questionnaire and EuroQoL-5D-5L (EQ-5D-5L) general health questi…

ROAD SAFETY PERFORMANCES FROM MICRO-SIMULATION CONSIDERING AUTONOMOUS VEHICLES OPERATIONS

2019

Introduction of Autonomous Vehicles in road traffic will remove the impact of the human factor from the driver’s decision-making process, and will transform the criteria for road network design, road infrastructure administration, traffic modeling and assessment tools for road safety management. The paper presents a microsimulation-based approach for safety performance evaluation of roundabouts where different percentages of Autonomous Vehicles are mixed with Conventional Vehicles. The simulated vehicle trajectories exported from VISSIM were used as a basis to estimate traffic conflicts through the Surrogate Safety Assessment Model (SSAM) for a sample of European roundabouts. 4. Inflammation, Endothelial Dysfunction and Arterial Stiffness as Therapeutic Targets in Cardiovascular Medicine.

2016

In the last decades, many factors thought to be associated with the atherosclerotic process and cardiovascular events have been studied, and some of these have been shown to correlate with clinical outcome, such as arterial stiffness, endothelial dysfunction and immunoinflammatory markers. Arterial stiffness is an important surrogate marker that describes the capability of an artery to expand and contract in response to pressure changes. It can be assessed with different techniques, such as the evaluation of PWV and AIx. It is related to central systolic pressure and it is an independent predictor of cardiovascular morbidity and mortality in hypertensive patients, type 2 diabetes, end-stage…

La prestación de maternidad en los supuestos de gestación subrogada

2018

Surrogate pregnancy, as a technique of assisted reproduction prohibited by Spanish law, raises a number of legal questions than, in certain cases, trascend of the Civil Law. Specifically, it has been discussed about de possibility that de person or persons consigner in the gestation contract substitution have access to the provision for maternity benefit. Recently, the Spanish Supreme Court has responded to this issue in the sense of recognizing the right to the provision for maternity benefit to the father or mother ?in fact?, efecttively, develop own parental powers.

Ethical challenges involved in obtaining consent for research from patients hospitalized in the intensive care unit

2017

International audience; Clinical research remains a vital contributor to medical knowledge, and is an established and integral part of the practice of medicine worldwide. Respect for patient autonomy and ethical principles dictate that informed consent must be obtained from subjects before they can be enrolled into clinical research, yet these conditions may be difficult to apply in real practice in the intensive care unit (ICU). A number of factors serve to complexify the consent process in critically ill patients, notably decisional incapacity of the patient due to illness or sedation. Obtaining consent for research from a designated proxy or family member, commonly termed a "surrogate de…

Measuring spectrally-resolved information transfer for sender- and receiver-specific frequencies

2020

AbstractInformation transfer, measured by transfer entropy, is a key component of distributed computation. It is therefore important to understand the pattern of information transfer in order to unravel the distributed computational algorithms of a system. Since in many natural systems distributed computation is thought to rely on rhythmic processes a frequency resolved measure of information transfer is highly desirable. Here, we present a novel algorithm, and its efficient implementation, to identify separately frequencies sending and receiving information in a network. Our approach relies on the invertible maximum overlap discrete wavelet transform (MODWT) for the creation of surrogate d…

A method for the time-varying nonlinear prediction of complex nonstationary biomedical signals

2009

A method to perform time-varying (TV) nonlinear prediction of biomedical signals in the presence of nonstationarity is presented in this paper. The method is based on identification of TV autoregressive models through expansion of the TV coefficients onto a set of basis functions and on k -nearest neighbor local linear approximation to perform nonlinear prediction. The approach provides reasonable nonlinear prediction even for TV deterministic chaotic signals, which has been a daunting task to date. Moreover, the method is used in conjunction with a TV surrogate method to provide statistical validation that the presence of nonlinearity is not due to nonstationarity itself. The approach is t…

2020

Abstract Background and Aims Hyperkalemia (HK) is a potentially life-threatening condition, in patients with chronic kidney disease (CKD) and congestive heart failure (CHF). The majority of patients affected with CKD or CHF, must be treated with inhibitors of renin angiotensin aldosterone system (RAASi) and mineralocorticoid receptor antagonists (MRAs). However, the treatments previously mentioned, increase the risk of HK episodes, which is the main cause of RAASi and MRAs downtitration or discontinuation, representing an undesirable clinical scenario, given that the patients are at high risk of be deprived of their nephroprotective effect and cardio-renal benefits The aim of the study is: …
