Recent data suggest that the traditional definition of bisphosphonate-associated osteonecrosis of the jaw (ONJ) may exclude patients who present with the non-exposed variant of the condition. To test the hypothesis that a proportion of patients with ONJ remain undiagnosed because their symptoms do not conform to the traditional case definition, we did a secondary analysis of data from MISSION (Multicentre study on phenotype, definition and classification of osteonecrosis of the jaws associated with bisphosphonates), a cross-sectional study of a large population of patients with bisphosphonate-associated ONJ who were recruited in 13 European centres. Patients with exposed and non-exposed ONJ…
AB1390 Osteoporosis associated morbidity analysis can reveal targets for better disease diagnosis and management
2018
Background Osteoporosis accounts for more disability and life years lost than rheumatoid arthritis. It brings an elevated risk of mortality, morbidity and re-/hospitalisation due to fractures and a significant financial and social burden. A rationale use of diagnostic and therapeutic resources is useful and should be encouraged at all intervention levels. For admitted patients, clinical risk factors for fractures and conditions that are causes for secondary osteoporosis should trigger either the diagnosis approach or the referral to a (Rheumatology/Endocrinology) specialist and treatment should be started during hospitalisation. Objectives We aimed to identify targets for educational or pro…
A rare case of infrarenal aortic coarctation in a young female
2020
Abstract Introduction Infrarenal abdominal aortic coarctation (AAC) is an extremely rare disease. It can be associated with renal artery stenosis determining secondary renal hypertension. Presentation of case We report a case of AAC in young female patient presenting systemic hypertension non-responder to medical treatment. Diagnostics revealed the involvement of the right renal artery as the cause of hypertension. The management consisted of percutaneous renal artery stenting and close surveillance for the aortic segment. The treatment was uneventful with resolution of the hypertensive condition. Discussion AAC etiology is unknown. There are no studies comparing the long-term treatment out…
EU-wide cross-sectional observational study of lipid-modifying therapy use in secondary and primary care: the DA VINCI study
2021
Abstract Aims To provide contemporary data on the implementation of European guideline recommendations for lipid-lowering therapies (LLTs) across different settings and populations and how this impacts low-density lipoprotein cholesterol (LDL-C) goal achievement. Methods and results An 18 country, cross-sectional, observational study of patients prescribed LLT for primary or secondary prevention in primary or secondary care across Europe. Between June 2017 and November 2018, data were collected at a single visit, including LLT in the preceding 12 months and most recent LDL-C. Primary outcome was the achievement of risk-based 2016 European Society of Cardiology (ESC)/European Atherosclerosis…
Gewicht und Funktion der Milz bei Polycythämie und Polyglobulie
1970
1. We have studied 50 patients with primary polycythemia by radioisotope scanning (“scintigraphy”). In 96% of these patients the weight of the spleen exceeded 300 g. The mean splenic weight was 720 g. 2. 93 % of 48 cases with secondary polycythemia revealed a mean splenic weight below 300 g, as detected by scintigraphy. The mean splenic weight in this group was 195 g. 3. The scintigraphic determination of splenic size allows polycythemia vera to be distiguished from secondary polycythemia with a high degree of accuracy
Dual-release hydrocortisone improves hepatic steatosis in patients with secondary adrenal insufficiency: A real-life study
2019
Background:Conventional glucocorticoid treatment has a significant impact on liver in patients with adrenal insufficiency. Dual-release hydrocortisone (DR-HC) provides physiological cortisol exposure, leading to an improvement in anthropometric and metabolic parameters. We aimed to evaluate the effects of 12-month DR-HC treatment on the hepatic steatosis index (HSI), a validated surrogate index of hepatic steatosis, in patients with secondary adrenal insufficiency (SAI).Methods:A total of 45 patients with hypopituitarism, 22 with hypogonadism, hypothyroidism, ACTH, and GH deficiencies, and 23 with hypogonadism, hypothyroidism, and ACTH deficiency, on replacement therapy for all the pituitar…
Higher levels of IgA and IgG at sepsis onset are associated with higher mortality: results from the Albumin Italian Outcome Sepsis (ALBIOS) trial
2021
Abstract Background The role of intravenous immunoglobulins (IVIG) during sepsis is controversial, as different trials on IVIG have observed inconsistent survival benefits. We aimed to elucidate the possible association and clinical significance between circulating levels of immunoglobulins. Methods In a subset of 956 patients with severe sepsis and septic shock of the multicentre, open-label RCT ALBIOS, venous blood samples were serially collected 1, 2, and 7 days after enrolment (or at ICU discharge, whichever came first). IgA, IgG and IgM concentrations were assayed in all patients on day 1 and in a subgroup of 150 patients on days 2 and 7. Ig concentrations were measured employing a tur…
One year after the ESC/EAS guidelines on cholesterol control. What's the new evidence? What's missing?
2021
The recent ESC/EAS 2019 Guidelines for the management of dyslipidaemias are centred on the causal role of low density lipoprotein (LDL), or more generally apolipoprotein B (apoB)-containing lipoproteins, in atherosclerosis as an essential principle. Despite updated goals and recommendations, that have further highlighted the importance of a powerful reduction in LDL-C levels to reduce the individual CV risk, some challenges remain to be addressed in view of future guideline elaboration. In this review, we will summarize the new evidence from clinical trials since 2019 guideline release and discuss the possible challenges for the future.
Antiplatelets in stroke prevention
2014
Stroke is the second cause of death worldwide and one of the leading cause of disability. Due to the high rate of recurrence, in high risk-patients (eg patients affected by atherosclerotic vascular disease), long-term antiplatelet therapy reduces the risk of vascular events such as non-fatal myocardial infarction, non-fatal stroke, or vascular death. The percentage of reduction of the events can be estimated in approximately 25%. These data justify the directions that are given to us by the current guidelines for prevention of secondary stroke, which recommend the broad use of antiplatelet therapy both for the secondary prevention of stroke in patients with a history of non-cardioembolic st…
Forms of Bullying and Associations Between School Perceptions and Being Bullied Among Finnish Secondary School Students Aged 13 and 15
2019
AbstractThe study aimed to examine the extent to which Finnish secondary school students experience bullying, how they are bullied, and whether being bullied is associated with school perceptions. The analyses were based on data from the Finnish part of the international Health Behaviour in School-aged Children (HBSC) study, and were obtained from 4262 students aged 13 and 15. The sample was nationally representative. Logistic regression analysis was performed to identify the associations between school perceptions and being bullied. Younger students reported being bullied more often than older students. Among younger students, boys were more often bullied than girls of the same age. There …
