Real-life impact of early interferon beta therapy in relapsing multiple sclerosis.

OBJECTIVE: Recent findings support greater efficacy of early vs. delayed interferon beta (IFNbeta) treatment in patients with a first clinical event suggestive of multiple sclerosis (MS). We aimed to evaluate the effectiveness of early IFNbeta treatment in definite relapsing-remitting MS (RRMS) and to assess the optimal time to initiate IFNbeta treatment with regard to the greatest benefits on disability progression. Opioid switching is often used to improve the opioid response in patients with cancer experiencing poor analgesia or adverse effects. When switching between drugs with delayed effect because of pharmacokinetics or type of delivery, concerns exist about the correct timing of introducing the second drug after stopping the previous one. There are sparse data for clinical management and cardiovascular outcomes in ARH. Effectiveness of opioid rotation in the control of cancer pain: The ROTODOL Study

Objective: To assess the effectiveness of opioid rotation (OR) to manage cancer pain. To describe the adverse events (AEs) associated with OR. Setting: Thirty-nine tertiary hospital services.Patients: Sixty-seven oncological patients with cancer-related pain treated at outpatient clinics.Intervention: Prospective multicenter study. The Italian Multiple Sclerosis Database Network (MSDN): The risk of worsening according to IFNβ exposure in multiple sclerosis

We evaluated the risk of worsening according to the length of exposure to interferon beta (IFNβ)ina large cohort of 2090 multiple sclerosis patients collected by the Italian MS Database Network. Overall 44-140 patient-visits with a follow-up of 22-143 patient-years were evaluated. Forty-one per cent of patients were exposed to IFNβ for up to 2 years, 39% for 2- 4 years and 20% for more than 4 years. A Cox regression model was used to analyse two clinical outcomes: disability progression and worsening of relapse rate. Primary motor area contribution to attentional reorienting after distraction

The anatomical structures involved in distraction-related processing in the auditory domain were investigated using magnetoencephalography. Participants performed a duration-discrimination task on a sequence of 200 and 400 ms long tones. Infrequent (12%) task-irrelevant pitch changes resulted in slower discriminative responses and more errors. Event-related potentials to these changes show an increased N1, a mismatch negativity, a P3a, and a reorienting negativity. Discourse processing depends on semantic memory as well as maintaining and updating of a mental model. Using event-related potentials, we investigated how a referent's information status (new, accessible, given) is processed in combination with three different prosodic realizations (an appropriate accent and two inappropriate accents). Visual distraction: a behavioral and event-related brain potential study in humans.

Recent studies reported that the detection of changes in the visual stimulation results in distraction of cognitive processing. From event-related brain potentials it was argued that distraction is triggered by the automatic detection of deviants. We tested whether distraction effects are confined to the detection of a deviation or can be triggered by changes per se, namely by rare stimuli that were not deviant with respect to the stimulation. Objective At a time of cost reduction in medical care efforts to manage the ever-increasing costs of new pharmaceutical drugs become increasingly important. Costs of four different muscle relaxant regimens including the new intermediate-acting neuromuscular blocking drugs (NMBD) cisatracurium and rocuronium will be analyzed.
