Predictors of Participation and Autonomy in People With Multiple Sclerosis.

2019

Abstract Importance: Because multiple sclerosis (MS) affects many life areas, it is important to know how participation and autonomy are associated with the perceived impact of MS on everyday life. Objective: To investigate how perceived quality of life, disease impact, gender, and disease severity predict participation and autonomy in people with MS. Design: Cross-sectional study in which structural equation modeling was used to evaluate relationships between measured variables. Settings: Outpatient clinics in three areas and one inpatient rehabilitation center in Finland. Participants: Convenience sample of 194 people with MS. Outcomes and Measures: Participants completed the Impact on Pa…

Interventions aimed to increase independence and well-being in patients with Alzheimer’s disease: Review of some interventions in the Italian context

2018

Abstract Background Alzheimer's disease (AD) represents a huge social and economic burden for the society and caring for individuals with AD is a complex and challenging task that requires a multidisciplinary approach. Studies have shown that several interventions provide beneficial results, but some non-pharmacological interventions have not yet been studied in depth and clinical trials using them are still pending. This review provides a summary of interventions based on paradigms aimed to increase independence and well-being in people with AD, as well as those interventions that decrease caregiver burden in long-term facilties for AD patients in Italy. Methods Narrative review regarding …

Norwegian municipal rehabilitation services in transition

2018

Background: Intending to meet increased demands of health care services due to the demographic change with an increasing elderly population, Norwegian municipalities along with many other western countries over the last couple of years have increased responsibility for providing rehabilitation services. The services include integrated homebased rehabilitation and institution- based rehabilitation services. The purpose of this study: How have workforces from the primary healthcare services experienced an increased responsibility for somatic rehabilitation? Methods: The study was based on qualitative research methods. We conducted five focus-group interviews with 25 participants with professi…

Beyond the lessons learned from the COVID-19 pandemic: opportunities to optimize clinical trial implementation in oncology.

2021

Abstract The COVID-19 pandemic affected millions of people globally with lasting effects on society, patients, investigators and health institutions. Clinical trials, our best tool to improve cancer treatment for patients through testing the clinical value of a new treatment, have been affected by the pandemic. The pandemic footprint represents both a risk of compromising development of new therapies and an opportunity to elicit discussion over a portfolio of broader reforms, applicable irrespective of pandemics, in order to improve the design and implementation of clinical trials in oncology. The administrative load should be reduced, without affecting the quality of research and principle…

Society for Immunotherapy of Cancer (SITC) clinical practice guideline on immunotherapy for the treatment of hepatocellular carcinoma.

2021

Patients with advanced hepatocellular carcinoma (HCC) have historically had few options and faced extremely poor prognoses if their disease progressed after standard-of-care tyrosine kinase inhibitors (TKIs). Recently, the standard of care for HCC has been transformed as a combination of the immune checkpoint inhibitor (ICI) atezolizumab plus the anti-vascular endothelial growth factor (VEGF) antibody bevacizumab was shown to offer improved overall survival in the first-line setting. Immunotherapy has demonstrated safety and efficacy in later lines of therapy as well, and ongoing trials are investigating novel combinations of ICIs and TKIs, in addition to interventions earlier in the course…

Cancer: New Needs, New Models. Is It Time for a Community Oncologist? Another Brick in the Wall

2021

Simple Summary Community care activity in the oncology field does not exist. This unmet need contrasts with the increasing number of patients with a previous diagnosis of cancer. Abstract Over the last few decades, thanks to early detection, effective drugs, and personalized treatments, the natural history of cancer has radically changed. Thanks to these advances, we have observed how survival of cancer patients has increased, becoming an ever more important goal in cancer care. Effective clinical governance of survivorship care is essential to ensure a successful transition between active and post-treatment life, identifying optimization of healthcare outcomes and quality of life for patie…

Effect of genetic variation in CYP450 on Gonadal impairment in a European cohort of female childhood cancer survivors, based on a candidate gene appr…

2021

Background: Female childhood cancer survivors (CCSs) carry a risk of therapy-related gonadal dysfunction. Alkylating agents (AA) are well-established risk factors, yet inter-individual variability in ovarian function is observed. Polymorphisms in CYP450 enzymes may explain this variability in AA-induced ovarian damage. We aimed to evaluate associations between previously identified genetic polymorphisms in CYP450 enzymes and AA-related ovarian function among adult CCSs. Methods: Anti-Müllerian hormone (AMH) levels served as a proxy for ovarian function in a discovery cohort of adult female CCSs, from the pan-European PanCareLIFE cohort (n = 743

Human papillomavirus genotype attribution in invasive cervical cancer: a retrospective cross-sectional worldwide study.

2010

Summary Background Knowledge about the distribution of human papillomavirus (HPV) genotypes in invasive cervical cancer is crucial to guide the introduction of prophylactic vaccines. We aimed to provide novel and comprehensive data about the worldwide genotype distribution in patients with invasive cervical cancer. Methods Paraffin-embedded samples of histologically confirmed cases of invasive cervical cancer were collected from 38 countries in Europe, North America, central South America, Africa, Asia, and Oceania. Inclusion criteria were a pathological confirmation of a primary invasive cervical cancer of epithelial origin in the tissue sample selected for analysis of HPV DNA, and informa…

Integration of oncology and palliative care: a Lancet Oncology Commission.

2018

© 2018 Elsevier Ltd Full integration of oncology and palliative care relies on the specific knowledge and skills of two modes of care: the tumour-directed approach, the main focus of which is on treating the disease; and the host-directed approach, which focuses on the patient with the disease. This Commission addresses how to combine these two paradigms to achieve the best outcome of patient care. Randomised clinical trials on integration of oncology and palliative care point to health gains: improved survival and symptom control, less anxiety and depression, reduced use of futile chemotherapy at the end of life, improved family satisfaction and quality of life, and improved use of health-…
