Sinusitis in Thyroid Associated Ophthalmopathy

In 19 patients with thyroid associated ophthalmopathy undergoing orbital decompression, histologic examination of paranasal sinus mucosa revealed moderate to severe inflammatory alterations in all patients. Only 2/19 patients had a history of chronic sinusitis. In 4/19 patients, mucosal abnormalities of the paranasal sinuses were found on preoperative computed tomography CT scans. Postoperatively, 10/19 patients developed purulent sinusitis. It is suggested that patients suffering from thyroid associated ophthalmopathy have an increased incidence of histologically detectable inflammatory changes of paranasal sinus mucosa. The Authors outline the law problems and the practice implications of thyroid and parathyroid surgery from the point of view of the informed consensus of the patient, and expose the criteria they usually adopt in their clinical practice for such procedures. This problem is particularly important because is exceptionally needed in emergency; therefore the preoperative information must be completed as possible, illustrating the risks of the procedure and the possible solutions alternative to surgery.

In particular, different social contexts can alter anxiety levels and modulate social behavior. The vertebrate-specific neuropeptide parathyroid hormone 2 (pth2) is directly regulated by the presence or absence of conspecifics in zebrafish. As its cognate receptor, the parathyroid hormone 2 receptor (pth2r), is widely expressed across the brain, we tested fish lacking the functional Pth2 peptide in several anxiety-related and social paradigms. Abstract Background The rapid intraoperative parathormone (PTH) and at central laboratory PTH dosage gives similar results. The central laboratory provides results in longer times and higher costs. Intraoperative measurement can reduce time and costs during parathyroidectomy. Methods Twelve patients undergoing parathyroidectomy for hyperparathyroidism renal transplant candidates were included. This report describes the use of intraoperative parathyroid hormone (ioPTH) assay during parathyroidectomy for patients with secondary hyperparathyroidism on the waiting list for renal transplantation. The levels of ioPTH were determined among waiting list patients undergoing subtotal parathyroidectomy and tertiary hyperparathyroidism patients undergoing procedures. The levels of ioPTH were significantly reduced at 10 minutes by 59.7,3% among with secondary hyperparathyroidism and 68.9% among tertiary hyperparathyroidism. Background: Calciphylaxis is a potentially fatal complication of persistent secondary hyperparathyroidism; its cause is still not clear. Unfortunately there is no close relation in severity of clinical picture, serological and pathological alteration. For this reason the prognosis is difficult to establish. Background. Quality of care and provider’s experience seem to be strictly connected in any field of surgery. Aim of this study is to identify a method to classify the centers on the basis of the number of thyroidectomies and parathyroidectomies performed. Methods. We listed 666 centers performing endocrine neck surgery in 2015, from the database of the Italian Health Ministry. We performed a descriptive statistic analysis with a dedicated software. Some authorities reserve the term for secondary hyperparathyroidism that persists after successful renal transplantation.
