CT-Angiographie an einem 16-Schicht-CT zur perioperativen Evaluation der Leberarterien im Vergleich zur MR-Angiographie

2004

Purpose: To evaluate the efficiency of CT angiography (CTA) with 16-row MSCT compared with MR angiography (MRA) in analyzing the arterial anatomy in patients undergoing liver surgery. Materials and Methods: In 30 patients, MRA and CTA studies of the abdominal vessels were reviewed. CT parameters: slice thickness 3 mm; collimation 1.5; reconstruction interval 2 mm (Philips MX 8000 IDT); 120 ml contrast media (400 mg/ml) at a rate of 4 ml/ sec; acquisition of arterial-phase scans. The anatomy of the hepatic artery was evaluated from axial and reconstructed maximum-intensity-projection (MIP) images ("Slab-Viewer", Philips). MR parameters: contrast-enhanced coronal FLASH-3D sequences; slice thi…

Mechanisms inactivating the gene for E-cadherin in sporadic gastric carcinomas

2006

Nonalcoholic fatty liver disease (NAFLD) is the most common cause for elevated liver enzymes in the developed nations. Beyond prevention programs which are of particular interest because of the increasing number of overweight children, treatment should be focussed on the most important risk factors, obesity and insulin resistance. As a consequence of elucidating the pathomechanisms of NAFLD, the number of potential therapeutic options increased. However, many studies investigating the therapeutic effect show shortcomings in at least one of the following points: lack of a serial liver biopsy, short term of treatment and limited number of included patients. The second generation insulin sensi…

Akute oder chronische Transplantat-Abstoßung? - HRCT des Thorax bei Patienten nach Lungentransplantation

2001

Purpose: Aim of the study was to evaluate the postoperative changes in patients with single (SLTX) or double lung transplantation (DLTX) with HRCT and to correlate those findings with the clinical diagnosis. Material and methods: 29 patients with SLTX (n = 14) or DLTX (n=15) were observed for more than 6 years after transplantation by HRCT (n =82). CT examinations were performed in inspiration and expiration (n=70) with a slice thickness of 1 mm and a feed of 10 mm. The image material was evaluated by 2 experienced radiologists in consensus. Criteria for acute rejection at HRCT were: ground glass opacities and focal air trapping in expiration. Criteria for chronic transplant rejection were:…

Use of Thromboelastography in severe sepsis a case-control Study

2011

Thromboelastography (TEG) is a global test of coagulation that records the viscoelastic changes in blood during clot formation. Cardiosurgery and liver transplantation are established fields of application for TEG. Severe sepsis is often characterized by an imbalance of the haemostatic equilibrium between clot formation and fibrinolysis in favor of a procoagulant status, especially in the first phase. A hypocoagulant status due to coagulation factor consumption could occur later. In spite of this, the correlation between TEG and sepsis is not clearly established. Moreover, there are doubts about which TEG-detected variable is best correlated with sepsis. The aim of this study is to clarify …

Formation of Venous Collaterals and Regeneration in the Donor Remnant Liver: Volumetric Analysis and Three-Dimensional Visualization

2009

Abstract Purpose We sought was to quantify and visualize the regeneration of the remnant liver after living donor liver transplantation using computed tomographic (CT) data. Methods For the evaluation of preoperative and follow-up data, we developed a software assistant that was able to compute the volume growth of the remnant liver and liver territories as well as visualize the individual growth of hepatic vessels over time. The software was applied to CT data of 20 donors who underwent right hepatectomy including the middle hepatic vein with at least 3 follow-up examinations in the first year after transplantation. Results After donation of a right lobe graft, the remnant liver regenerate…

Antimicrobial prophylaxis in allogeneic bone marrow transplantation. Guidelines of the Infectious Diseases Working Party (AGIHO) of the German Societ…

2005

Patients undergoing allogeneic stem cell transplantation are at high risk for infection with a variety of pathogens during different phases of the procedure. Bacteria and fungi predominate the first phase until engraftment. During the second phase, from engraftment to about day 100, major infectious problems are caused by fungi and cytomegalovirus. Both pathogens remain important under continued immunosuppression, however, in the late post-transplantation period infections with encapsulated bacteria may become a problem. In this review the Infectious Diseases Working Party of the DGHO gives recommendations for prophylaxis of infections under allogeneic stem cell transplantation with drugs a…

Acellular dermal matrix in soft tissue reconstruction prior to bone grafting. A case report.

2010

When hard tissue augmentation is scheduled as a part of an oral rehabilitation, prior to the treatment, it is important to assess if the quality of the underlying gingiva at the recipient site can support the bone grafting procedure. The most frequent complication during autologous onlay grafts are wound dehiscences in the recipient site, so the integrity of soft tissues is a basic aspect of successful reconstructive and plastic surgical procedure. Connective tissue grafts can improve the quality and quantity of soft tissue in oral sites where a hard tissue reconstruction is going to take place. However, particularly when large grafts are harvested, the autogenous donor site can present sig…

Fat and liver transplantation: clinical implications

2018

Nonalcoholic steatohepatitis (NASH), with or without hepatocellular carcinoma, is a growing indication for liver transplantation (LT) worldwide, particularly in the Western world. Patients with NASH typically combine features of metabolic syndrome with cardiovascular comorbidities, which challenge pre-LT evaluation, surgical approaches, post-LT management, and outcomes. Post-LT survival in NASH patients is excellent, similar to that achieved with other indications, particularly in the absence of cardiovascular comorbidities. Although disease recurrence on the liver allograft is common, progression to advanced disease is uncommon, at least in the short term. Whether this holds true with long…
