Electrophysiologic evaluation of phrenic nerve and diaphragm function after coronary bypass surgery: Prospective study of diabetes and other risk fac…

2006

Objective Phrenic neuropathy after coronary artery bypass grafting has been related to various risk factors with conflicting results. The aim of this study was to assess the incidence, characteristics, and clinical consequences of phrenic neuropathy and the influence of diabetes and other risk factors. Methods We conducted an observational, prospective study of parallel groups including 94 consecutive patients subjected to coronary artery bypass grafting, half of them with diabetes and associated polyneuropathy . Electrophysiologic study of phrenic nerve conduction as the reference method, chest radiography, diaphragm ultrasound, and functional respiratory tests were performed 24 to 48 hour…

Efficacy of coronary revascularization in patients with acute chest pain managed in a chest pain unit.

2009

To investigate the safety of discharge of patients deemed at low risk of cardiac events after evaluation in a chest pain unit and to determine the prognostic effect of revascularization of patients deemed at high risk. The study population consisted of 1088 patients presenting at the emergency department from January 15, 2001, to September 1, 2006, with chest pain but without ischemia on electrocardiography or troponin elevation. Patients were managed by a chest pain unit protocol that included early exercise testing. Three groups of patients were distinguished: (1) those discharged after exercise testing (424 [39%]); (2) those in whom unstable angina was ruled out after in-hospital evaluati…

Innovative fully robotic 4-arm Ivor Lewis esophagectomy for esophageal cancer (RAMIE4).

2019

Nowadays robotic surgery is established for abdominal and thoracic surgery. It has been shown that complex procedures are feasible using robotic systems, e.g., da Vinci Xi, with a huge benefit in precision. Different techniques for esophageal cancer surgery are reported; however, only a few robotic and partial robotic procedures are described. Therefore, a fully robotic (abdominal and thoracic) Ivor Lewis esophageal resection using four robotic arms-RAMIE4-the standard technique used for lower esophageal cancer, is presented in this paper. The technique shown in the video was performed successfully in 100 cases in 24 months. The reconstruction is performed with a gastric conduit pull-up and…

Diagnosis of papillary muscle rupture after acute myocardial infarction by transthoracic and transesophageal echocardiography.

1993

The sensitivity of transthoracic echocardiography to visualize the structural abnormality of papillary muscle rupture (PMR) after acute myocardial infarction can be anticipated to average about 50%; therefore, we evaluated five patients exhibiting the condition with both transthoracic and transesophageal echocardiography. The use of the two imaging techniques resulted in the fact that no instance of PMR was missed. Using transthoracic echocardiography in two patients and transesophageal echocardiography in four, the ruptured papillary muscle was visualized directly. Mitral insufficiency as an indirect sign was observed in all patients. In one patient the papillary muscle rupture developed i…

Head, chest and femur injury in teenage pedestrian–SUV crash; mass influence on the speeds

2018

This work studies the teenage pedestrian–sport utility vehicle (SUV) crash; injury to the vital parts of the body, such as the head and chest, and to the femur is evaluated. More advanced injury criteria are applied, as provided in the rules. The multibody technique is applied by making use of SimWise software and of the teenager anthropomorphic model, the use of which is now consolidated. Head injury criterion (HIC) is used for the head, thoracic trauma index (TTI) criterion for the thorax in the case of side impact and 3 ms criterion in the case of frontal impact, while the force criterion is used for the femur. Both the TTI and femur load evaluation require non-substantial modifications…

487 The Influence of Inhaled Corticotherapy on the Growth and Development in Asthmatic Children

2012

Background Inhaled corticotherapy is the main anti-inflammatory controller type therapy in asthmatic children. Impaired growth as a result of long-term corticotherapy remains a disputed issue of topical interest for both endocrinologists, pneumologists and pediatricians. Objective The study evaluates the influence of long term small dose inhaled corticotherapy on growth and somatic development in asthmatic children. Methods Observational analytical study on 2 samples of subjects. The study group: 100 asthmatic children with small dose inhaled corticotherapy (beclomethasone dipropionate Becotide 200–400 µg/day or fluticasone propionate Flixotide 100–300 µg/day), for 24 months therapy. Available evidence on re-irradiation with stereotactic ablative radiotherapy following high-dose previous thoracic radiotherapy for lung malignancies

2015

Patients affected with intra-thoracic recurrences of primary or secondary lung malignancies after a first course of definitive radiotherapy have limited therapeutic options, and they are often treated with a palliative intent. Re-irradiation with stereotactic ablative radiotherapy (SABR) represents an appealing approach, due to the optimized dose distribution that allows for high-dose delivery with better sparing of organs at risk. This strategy has the goal of long-term control and even cure. Aim of this review is to report and discuss published data on re-irradiation with SABR in terms of efficacy and toxicity. Results indicate that thoracic re-irradiation may offer satisfactory disease c…

Abdominal and Thoracic Magnetic Resonance Angiography: Challenges, Technical Approaches, and First Clinical Applications

1993

The main goal of magnetic resonance angiography (MRA) is to enrich the huge potential of magnetic resonance imaging (MRI) by additional morphological and functional information about the vasculature. In spin echo sequences fast flowing blood leads to signal extinction, providing high contrast between tissue and vascular structures without the need for contrast media. However, various artifacts are frequently observed which alter the assessment of the vasculature. They are due to alterations of flow (slow velocity or turbulences) or induced by the acquisition technique (Mills et al. 1983; Bradley and Waluch 1985). By using gradient echo sequences, high-quality angiograms can be obtained from…

Positronen-Emissions-Tomographie zum präoperativen Staging des Ösophaguskarzinoms

2002

BACKGROUND AND OBJECTIVE Exact preoperative staging is a prerequisite for the indication as well as the choice of the appropriate operative technique for patients with esophageal carcinoma. In this prospective study we assessed whether positron emission tomography (PET) with (18)F-fluorodeoxyglucose (FDG) increases the accuracy of staging compared to standard computer tomography (CT) and leads to a different therapeutic approach. PATIENTS AND METHODS 30 patients (25 men, 5 women, median age 63 years) with histologically identified carcinoma of the esophagus (n =29) and the cardia (n = 1), respectively, were studied. All patients underwent FDG-PET imaging of the neck, chest, and abdomen as w…
