Induced post-traumatic apexification: 20 year follow-up and morphological study after new fracture.

Apexification is an endodontic technique used in immature necrotic teeth, the primary goal of which is to induce a calcified barrier in roots with open apices and to promote the continuation of apical root development. A 7-year-old patient with a coronal fracture in a maxillary central incisor was treated with an apexification technique using calcium hydroxide. The apical closure was obtained after one year, then a permanent root canal filling was performed. The tooth was monitored for a period of 20 years, up to when a trauma caused its extraction. An Ex Vivo Study of Root Canal System Configuration and Morphology of 115 Maxillary First Premolars

Abstract Introduction The aim of this study was to investigate the root canal system morphology of maxillary first premolars by means of micro–computed tomographic imaging in a Swiss-German population. Methods The root canal configuration (RCC) of 115 maxillary first premolars (Mx1Ps) were investigated by means of micro–computed tomographic imaging and 3-dimensional imaging. The RCC and the physiological foramina results are described by a 4-digit system code. Results Twelve different RCCs were observed in 30 single-rooted Mx1Ps; 2-2-2/2 (30.0%), 1-2-2/2 (13.3%), 1-2-1/2 (10%), and 2-2-1/2 (10.0%) were the most frequent ones. Dental findings and orthopantomograms of 984 patients (aged 20–60 years; 524 females/460 males) were analyzed. Failure of endodontic treatment due to a palatal gingival groove in a maxillary lateral incisor with talon cusp and two root canals.

A case is presented in which endodontic treatment of a maxillary lateral incisor with a talon cusp and two root canals failed following a mistaken diagnosis. What was first diagnosed as an endodontic lesion was, in all probability, a primary periodontal lesion caused by the advance of bacteria from the gingival crevice to the apex along the radicular groove between the main tooth and the talon cusp.
