It has been demonstrated in an experimental study in swine using the scanning electron microscope that a rebreathing technique utilising minimal fresh gas flowrates significantly improves climatization of anaesthetic gases. Consequently, effects of various anaesthetic techniques on airway climate must be assessed, which covers the need for suitable measuring devices. To assess the feasibility of helium-3 magnetic resonance (MR) imaging with a three-dimensional fast low-angle shot (FLASH) sequence, He-3 gas (volume, 300 mL; pressure, 3 x 10(5) Pa; polarized up to 45% by means of optimal pumping) was inhaled by five healthy volunteers and five patients with pulmonary diseases. All breath-hold examinations (22-42 seconds) were completed successfully. Normal ventilation was depicted with homogeneous high signal intensity, lesions were depicted as causing defects, and obstructive lung disease was depicted with severely inhomogeneous signal intensity.

Background: Despite different support techniques, the surgical management of tracheomalacia is still a challenging problem. Satisfactory results after internal stenting are above 80%, whereas, when performing external stenting using biocompatible ceramic rings, results are reported at over 90%. Tracheomalacia resulting from tracheostomy or compressive thyroid disease often represents a difficult problem in airway management. In an attempt to improve this condition, biocompatible ceramic rings were surgically implanted in 16 patients to restore normal patency of the airway by first expanding the tracheal lumen lateral, and then in an anterior dimension. Preoperatively, patients displayed moderate to severe obstruction with marked restrictions in lifestyle, as confirmed by history, physical examination, and airway resistance studies. The EasyTube (EzT) is a new sterile, disposable airway device approved by the European Union in February 2003 and by the U.S. Food and Drug Administration in January 2005. Rupture of supra-aortic neck arteries due to lesions caused by tracheal tubes

Haemorrhages from the common carotid or innominate artery caused by tracheal tubes are rare but dramatic complications of head and neck surgery. Patients with metal tracheostomy cannulas or patients in the phase of wound healing after radical tumour resections of the pharynx and larynx run a greater risk. Following extensive tumour resections and radiation of the upper respiratory and digestive tracts in 680 patients, acute bleeding of the larger neck arteries occurred in 18 patients. In four of these cases, fistulae developed in the innominate or common carotid artery. These ruptures were caused by both plastic and metal tracheal tubes.
