Electrophysiological brainstem testing in the diagnosis of reversible brainstem ischemia.

The aim of this study was to evaluate the sensitivity of multimodal electrophysiological brainstem testing in the diagnosis of clinically suspected reversible ischemic deficits of the brainstem compared with diffusion weighted MR imaging. We investigated 158 consecutive patients presenting with signs of acute brainstem dysfunction. Serial electrophysiological brainstem tests including masseter reflex, blink reflex, masseter inhibitory reflex, AEP, MEP, EOG and the oculoauricular phenomenon were applied. We report an unusual case of a 35-year-old Ivorian migrant with an abdominal mass and medical history relevant for human immunodeficiency virus-2 positivity with a CD4/CD8 ratio of 0.63; Mantoux and lymphocyte stimulation tests (QuantiFERON) were positive. 3D-CT images revealed a voluminous non-homogeneous retroperitoneal mass in the right abdominal region presenting no significant contrast impregnation. A meshless particle method based on an unconditionally stable time domain numerical scheme, oriented to electromagnetic transient simulations, is presented. The proposed scheme improves the smoothed particle electromagnetics method, already developed by the authors. The time stepping is approached by using the alternating directions implicit finite difference scheme, in a leapfrog way. The proposed formulation is used in order to efficiently overcome the stability relation constraint of explicit schemes. A computer-aided simulation technique for the dynamical analysis of a class of GaAs FET optically-driven injection-locked oscillators (ODILO's) is presented. By combining a measurement-derived nonlinear model of the illuminated transistor with a phasor-domain analysis method, a first-approximation - but phenomenologically complete - differential model of the synchronized oscillator is derived.
