In clinical practice, the clinician is challenged with symptoms and/or signs at times apparently insoluble by diagnostic and/ or therapeutic means. We propose that in these cases, we have to use an EBM approach in which evidence may be looked up in every available clinical report and bibliographic databases are used for searching that evidence. A decision tree to help determine the best timing and antiretroviral strategy in HIV-infected patients.

SUMMARYOptimal antiretroviral strategies for HIV-infected patients still need to be established. To this end a decision tree including different antiretroviral strategies that could be adopted for HIV-infected patients was built. A 10-year follow-up was simulated by using transitional probabilities estimated from a large cohort using a time-homogeneous Markov model. The desired outcome was for patients to maintain a CD4 cell count of >500 cells/mm3without experiencing AIDS or death. The case of a 43-year-old woman with intralobar pulmonary sequestration, Pryce type one, is presented. The medical history was characterised by recurrent bronchopneumonia, productive cough with purulent sputum and hemoptysis in the last three years. Diagnosis was made by CT angiography: multiplanar, maximum intensity projection and volume rendering reconstructions were visualised. Background: To analyse the discriminative impact of osteopontin (OPN) and activated leukocyte cell adhesion molecule (ALCAM), combined with human epidermal growth factor 2 (HER2) and oestrogen receptor (ER) in breast cancer. Methods: Osteopontin, ALCAM, HER2 and ER mRNA expression in breast cancer tissues of 481 patients were analysed (mRNA microarray analysis, kinetic RT–PCR). Since it was introduced in the 1990s, axillary sentinel lymph-node biopsy has been rapidly and widely adopted to avoid complete axillary dissection (though this is still the standard procedure). The aims of the study were two-fold: (i) to determine the value of different techniques of sentinel lymph-node identification and (ii) to verify the predictive value of such procedures through histological examination of the sentinel lymph node and axillary dissection in the same patients. Arbuscular mycorrhizal fungi (AMF) may promote plant invasion by enhancing plant performance and competitiveness. However, only a small number of studies have considered the interactions between local soil microbial communities and invasive plants, and even fewer have focused on alien trees. Ailanthus altissima is a serious problem in the Mediterranean Basin, where it has invaded many habitats. We investigated the symbiosis between A. altissima and indigenous AMF in two invaded, ecologically different Mediterranean woodlands. Con las primeras elecciones municipales democráticas en España después del franquismo, los nuevos ayuntamientos establecieron programas para reconstruir las ciudades españolas. Un aspecto de estos proyectos fue la creación de espacios públicos donde la gente pudiera encontrarse en un ambiente agradable. Estas áreas se cerraron al tránsito y se amueblaron con árboles, bancos y un nuevo tipo de esculturas artísticas. En la zona de Alicante, el ceramista Arcadio Blasco, ya conocido en Madrid y en Roma, creó cinco monumentos urbanos de gran escala.
