Summary Aims:  The choice of insulin at initiation in type 2 diabetes remains controversial. The aim of this study was to assess the occurrence of self-reported severe hypoglycaemia associated with premixed insulin analogues in routine clinical care. Methods:  A 12-month, prospective, observational, multicentre study in patients starting a commonly prescribed premixed insulin analogue (either insulin lispro 25/75 or biphasic insulin aspart 30/70, twice daily) after suboptimal glycaemic control on oral antidiabetic agents. Persons with type 2 diabetes mellitus (DM2) have an increased incidence of cognitive decline and dementia. An increased cortical and subcortical atrophy has been found after controlling for vascular disease and inadequate cerebral circulation. A possible role of insulin resistance and hyperinsulinemia has been suggested to mediate the link between DM2 and Alzheimer’s Disease (AD). Obstructive sleep apnea (OSA) is characterized by upper airway collapse during sleep. Chronic intermittent hypoxia, sleep fragmentation, and inflammatory activation are the main pathophysiological mechanisms of OSA. OSA is highly prevalent in obese patients and may contribute to cardiometabolic risk by exerting detrimental effects on adipose tissue metabolism and potentiating the adipose tissue dysfunction typically found in obesity.
