The hemodynamic and anti-ischemic effects of nitroglycerin (NTG) are rapidly blunted as a result of the development of nitrate tolerance. With initiation of NTG therapy, it is possible to detect neurohormonal activation and intravascular volume expansion. These so-called pseudotolerance mechanisms may compromise the vasodilatory effects of NTG. Long-term nitrate treatment also is associated with decreased vascular responsiveness caused by changes in intrinsic mechanisms of the tolerant vasculature itself. Abstract 1 In isolated, electrically driven (3 Hz) hearts of guinea-pigs the action of prostaglandin E2 on left ventricular pressure (LVP), oxygen consumption (Qo2) and coronary vascular resistance (CVR) was studied by establishing cumulative concentration-response curves. Left ventricular pressure (LVP), left ventricular pressure derivative (LV dp/dtmax), coronary vascular resistance (CVR) and myocardial oxygen consumption (Qo2) were measured simultaneously in isolated, electrically driven hearts of guinea-pigs at constant perfusion rate. 2 LVP, LV dp/dtmax, CVR and Qo2 were greatly decreased by either the addition of prostaglandin E2 (50 ng/ml) to the perfusion fluid or pretreatment of the animals with reserpine. 3 Isoprenaline (0.5 nM to 100 nM) induced increases in LVP, LV dp/dtmax and Qo2. INTRODUCTION There is little information regarding the comparative hemodynamic effects of adding milrinone or levosimendan to dopamine infusion in hypoxia-reoxygenated (H-R) newborns. RESULTS Severely hypoxic piglets had cardiogenic shock with depressed cardiac index (CI) and mean arterial pressure (MAP). The hemodynamics deteriorated gradually after initial recovery upon reoxygenation. Heart rate and CI improved with milrinone (D+M) and levosimendan (D+L) administration (P < 0.05 vs. control). Unilateral renal blood flow was evaluated in-situ in 13 dogs by cineangiodensitometry and microsphere distribution studies before and after intermittent balloon occlusion with and without hypothermic perfusion of one kidney. The contralateral kidney served as control. No significant difference in renal blood flow and vascular resistance was noted before, and 5, 30 and 60 minutes after unilateral intra-arterial manipulation. Nicorandil is a potent vasodilator with antianginal and anti-ischemic properties that acts on both the coronary and the peripheral vascular bed. Because of its dual vasodilatory mechanisms mediated by an increase in cyclic GMP similar to that of nitrates and by a selective increase in the K+ conductance of the smooth muscle cell membrane, nicorandil unloads the right and left ventricles at rest and during exercise. In vivo-Untersuchungen der Tumordurchblutung an Implantationstumoren eines DS-Carcinosarkoms in der Rattenniere zeigen eine deutliche Abhangigkeit der Durchblutungsgrosse vom Tumorgewicht. Bei Variation des arteriellen Mitteldrucks zwischen 40 und 135 mm Hg steigt die Durchblutung der einzelnen Tumoren linear mit zunehmendem Perfusionsdruck an. Eine zunehmende Rarefizierung der terminalen Strombahn mit ansteigendem Tumorgewicht wird belegt durch eine starke Zunahme des Stromungswiderstandes, der schon bei sehr jungen Tumoren wesentlich hohere Werte als bei verschiedenen Organen aufweist.

Experiments are performed to study the influence of local hemodilution on tumor blood flow, oxygen availability in tumor tissue and O2 consumption of cancer cells. The results obtained clearly show that hemodilution in isolated tumor perfusion can distinctly improve nutritive blood flow through solid tumors. This can be utilized to enhance pharmacokinetics of antitumor drugs. Due to the improved metabolic status, the pharmacodynamics of some antitumor drugs should also be enhanced. To achieve a maximum improvement of the O2 supply to the tumor, hematocrit values should not be decreased below 0.20.
