Chronic cerebrospinal venous insufficiency: current perspectives

This review summarizes the research to date on chronic cerebrospinal venous insufficiency (CCSVI). CCSVI was initially defined as a clinical syndrome comprising stenoses of the internal jugular and/or azygos veins, characterized by collateral venous outflows and reduced cerebral blood flow, and was found primarily in patients with multiple sclerosis. However, the published evidence on CCSVI is very discordant. Catheter venography studies gave a regular picture, with the majority of patients with multiple sclerosis presenting with demonstrable outflow abnormalities in the veins draining the central nervous system. Radiosurgical Treatment of low flow Carotid-Cavernous Fistulae

The good results obtained by stereotactic radiosurgery in arteriovenous malformations has led the authors to expect similar results in low flow carotid-cavernous fistulae. In this paper, 20 cases who underwent radiosurgery with a conventional gamma source are presented. The total dose delivered was 36 to 40 Gy. 90% of the patients were cured after radiosurgery after a mean time of 7 months. Those presenting a mild improvement after a mean time of 2 months and those with a marked improvement after 4 months.

Arteriovenous malformations (AVMs) are a rare congenital vascular disorder. They represent a fast-flow vascular malformation. Clinically, AVMs present a heterogenous expression and can affect every part of the body. Here, we will solely focus on extracranial AVMs. Generally, AVMs progress with the patient's age. Patients often suffer from pulsation, skin discoloration, pain, ulceration, bleeding, and disfigurement. Diagnostic tools include color-coded duplex sonography, MRI and CT imaging, as well as the clinical examination. 4D dynamic perfusion-computed tomography may help in the interventional planning. Massive glosso-cervical arteriovenous malformation: The rationale for a challenging surgical resection

Massive arterivenous malformations (AVM) in the cervico-facial area are rare but potentially life-threatening. Treatment protocols are not well-established. A 41-year old man presented large painless rubber-like mass within the entire neck, which also extended intraorally through the floor of the mouth, showing a slow growing pattern for 5 years. Angiography diagnosed it as cervicofacial AVM. Treatment approach consisted on the embolization of the right upper thyroid, lingual and facial arteries under intravenous sedation.
