The Economic Burden of Child Maltreatment in High Income Countries.

2015

Maltreatment is a common cause of children’s functional and emotional impairment. Costs for the society are high, as a substantial amount of resources have been allocated for various types of services connected to maltreatment of children. These include acute treatment, long-termcare, family rehabilitation programs, and judiciary activities. There is a long-lasting debate on how child abuse could be prevented or reduced. How can the costs of related services be contained? What is the role of pediatricians in such efforts? This article raises these important questions within the framework of the debate opened by the article by Gerber-Grote et al regarding the role of health economics in impro…

Battered child syndrome: cerebral ultrasound and CT findings after vigorous shaking.

1992

Child abuse by whiplash-shaking can lead to severe cerebral damage, neurological defects and mental retardation. Cerebral damage has been found with and without external evidence of head injury. We report the sonographic findings in two children after traumatization due to repetitive vigorous whiplash shaking. Cerebral sonography revealed cerebral edema at admission or within 48 hours thereafter. Follow-up studies demonstrated development of marked brain atrophy in both cases. The sonographic findings were confirmed by cranial computerized tomography. Doppler sonography was used to monitor cerebral perfusion by measuring intracranial blood flow. The clinical history of the patients demonstr…

Mothers' abusive childhood predicts child abuse

1999

To examine the intergenerational transmission of abuse, the study compared 25 mothers whose child had been under the supervision of the child protection services (CPS) with 25 mothers who had had no contact with the CPS. The groups were compared with respect to their own self-reported childhood abuse, their abuse of their own child and punitiveness. The data were gathered using interviews and files. There were no significant differences between the groups in self-reported childhood physical abuse, but the CPS mothers had experienced more childhood psychological abuse, especially rejection, accusations, terrorizing and corrupting. The groups did not differ in the self-reported physical or ps…

Lichen sclerosus Mistaken for Sexual Abuse – Prevention of Victimization

2014

This case report describes the forensic examination of 7 girls aged between 4-11 years with suspected sexual abuse. The children had been reported by local pediatricians, hospital physicians or authorities such as the youth welfare office and police due to mucosal changes in the anogenital area. All children presented with a clearly delimited, porcelain-like white discoloration of the skin in the anal and genital area with atrophic conversion to parchment-like skin. The markedly vulnerable skin exhibited very different manifestations of hemorrhagic dermal and mucosal defects in the area of the labia, the clitoris, the perineum and the perianal region. The hymen was intact in all cases. The …

Improving child maltreatment detection systems: a large-scale case study involving health, social services, and school professionals.

2003

Abstract Objectives: The purpose of this 5-year study was to improve detection in two consecutive phases: (a) To close the gap between the number of identified cases and the actual number of cases of child abuse by increasing detection; and (b) To increase the possibility of a broader spectrum of detection. Method: The Balearic Islands (one of the Autonomous Communities of Spain, with 161,287 children under 18 years old) was selected as the study area. Phase 1: front-line professionals (181) from all the health and social services agencies were trained in detection. Phase 2, school professionals (251) from all schools in the territory were also trained. The independent factor was the interv…

2005

BACKGROUND In recent years, the long-term survival of childhood cancer patients has increased considerably. While this is desirable, more patients with late effects are to be expected and studies thereof become increasingly important. We will need to be able to stay in touch with as many former patients as possible in order to make a systematic and comprehensive long-term follow-up possible. PATIENTS Childhood cancer patients under 15 years of age at diagnosis resident in Germany and registered at the German Childhood Cancer Registry (GCCR). METHODS The GCCR has established a 3-phase procedure for follow-up. We developed principles for the long-term follow up of these patients. They are bas…

Plasma non-cholesterol sterols: a useful diagnostic tool in pediatric hypercholesterolemia.

2010

Current guidelines strongly recommend the identification of genetic forms of hypercholesterolemia (HC) during childhood. The usefulness of non–cholesterol sterols (NCS) in the diagnosis of genetic HC has not been fully explored. Plasma NCS were measured by gas chromatography/mass spectrometry (GC/MS) in 113 children with hypercholesterolemia affected by: autosomal dominant hypercholesterolemia (ADH), familial combined hyperlipidemia (FCHL), polygenic hypercholesterolemia (PHC), and in 79 controls to evaluate: i) plasma NCS profile in different genetic HC and ii) the usefulness of NCS for the diagnosis of HC beyond current clinical criteria. ADH was characterized by raised lathosterol/total …
