Oral manifestations as the first presenting sign of Crohn?s disease in a pediatric patient

Crohn’s disease (CD) is a chronic inflammatory disorder affecting the gastrointestinal (GI) tract. Although the GI tract is the primary site of involvement, many patients, particularly in pediatric cases, first present with non-intestinal manifestations, including oral lesions. Oral manifestations of CD in children occur in around 50-80% of cases, and about 30% of CD cases in children occur first in the mouth. Recognizing such oral lesions in the pediatric population, and requesting a biopsy, may expedite the diagnosis of CD. Laryngeal manifestations of Crohn's disease

Crohn’s disease is a chronic inflammatory bowel disease. It should be considered a multisystemic disease due to its extra-intestinal manifestations found in 25-40% of patients with inflammatory bowel disease. Laryngeal involvement is very uncommon and the lesions in hypopharynx and larynx are edema, ulcerations and granulation tissue. Symptoms are dysphonia, dysphagia, odynophagia and airway restriction. Crohn's disease (CD) is a type of inflammatory bowel disease (IBD) and its etiology is multifactorial and involves a combination of genetic and environmental factors. The interaction of these factors causes an imbalance in the microbiota, leading to the activation of several immunological and inflammatory mechanisms. From an immunological point of view, there seems to be an involvement of the TIM-3/galectin-9 pathway and of the autoregulation of LyTh1. The studies show that in patients with CD the autoregulation of LyTh1 is lost due to a reduced concentration of galectin-9 and a reduced TIM-3 expression in LyTh1.
