Obstructive sleep apnea (OSA) is associated with cardiovascular diseases. Aim: To analyze structural parameters of myocardial damage in patients presenting with acute coronary syndrome (ACS) and its relationship with OSA severity. Method: This is a prospective, observational study in patients with a first acute myocardial infarction (MI) treated by percutaneous coronary intervention (PCI). Cardiovascular magnetic resonance (CMR) studies were performed at 3 days and 6 months. Eligible patients underwent a polysomnography within 30 days after PCI. Setting: Department of Ophthalmology and Institute of Medical Physics, University of Vienna, Vienna, Austria, and Department of Ophthalmology, University of Mainz, Mainz, Germany. Dual lumen microcatheters (DLMC) have become indispensable tools in the setting of percutaneous coronary intervention (PCI) of chronic total occlusion (CTO). Other than allowing preservation and treatment of bifurcated coronary branches within or in the proximity of the CTO-body, they enable the use of modified parallel wiring, antegrade dissection and re-entry, collateral selection and retrograde negotiation of the distal CTO-cap. This Euro-CTO consensus document describes current DLMCs and suggests a practical guide to anatomies and techniques in which these devices are applicable.

Dual-release hydrocortisone vs conventional glucocorticoids in adrenal insufficiency

Background Dual-release hydrocortisone (DR-HC) improves metabolism in patients with adrenal insufficiency. The aims of this study were to compare the cardiovascular and metabolic effects of conventional glucocorticoids (GCs) vs. DR-HC and of high vs. low doses of GCs, after 48 months of observation. Methods We selected 27 patients on hydrocortisone (mean dose 17.5 ± 4.2 mg/day) and 20 patients on cortisone acetate (mean dose 37.5 ± 12.1 mg/day) who maintained this treatment (group A) and 53 patients switched to DR-HC (mean dose 22 ± 4.8 mg/day) (group B). Gastroduodenal perforation is an emergency situation that usually requires early recognition and well-timed surgical treatment. It can arise from different natural, iatrogenic, or traumatic causes, and it can present with various symptoms especially in the early phase. This article reviews the role of the different imaging techniques in the diagnosis of gastroduodenal perforation, focusing on the direct and indirect signs that are encountered in conventional radiography and computed tomography; our personal experience is also provided.
