Objectives: A study is made of corticosteroid use in controlling pain, swelling and trismus after lower third molar surgical removal. Material and methods: A Medline search and review was made of all randomized and controlled clinical trials on the effects of corticosteroids upon pain, swelling and trismus after lower third molar surgery. The review was limited to those studies published in the last 10 years. Of the 20 initially considered articles, 6 were finally excluded. Results: A total of 14 articles were included. Adrenal myelolipoma associated with adenoma

The association of an adrenal myelolipoma with a non-functioning adenoma is very rare. Herein, we report on such a case in an asymptomatic 64-year-old woman. To the best of our knowledge, there is only one other case of non-functioning adrenocortical adenoma associated with myelolipoma in the same gland. Furthermore, only two other adenomas (mineral corticoids and corticosteroid producing tumors) associated with myelolipomas have been reported, and both myelolipomas were less than 1 cm in size. In conclusion, the 8.5 cm myelolipoma in our case is the largest compared with the three previously reported ones.

Selective nerve root block by a transforaminal approach with radiological control allows injection of an appropriate corticosteroid next to the nerve root, although it has been associated with major neurological complications in a few cases. There have been several reports in which corticosteroid injections performed at the level of the cervical or lumbar spine have led to severe spinal cord ischemic infarction. These have occurred independently of whether the procedure was performed under radiological control, in which the tip of the needle is identified to prevent vascular spreading of the solution injected.
