Refractory coeliac disease: one step closer to the origin of aberrant lymphocytes.

2012

Aberrant intraepithelial lymphocytes (IELs) are the key feature of refractory coeliac disease type II (RCDII), but the cellular origin of these aberrant IELs remains unclear. In a recent paper Schmitz et al 1 further explored previously characterised aberrant IEL cell lines from four patients with RCDII by using a broad spectrum of cell specific markers, RNA array and immunological techniques, to compare these cell lines to IELs from the fetal intestine, the intestine of children and adults and the thymus. IELs are a heterogeneous population of lymphocytes with innate and adaptive features that inhabit the small and large intestine. IELs are important for the maintenance of tolerance to com…

Non-celiac wheat sensitivity: rationality and irrationality of a gluten-free diet in individuals affected with non-celiac disease: a review

2021

AbstractNon-celiac gluten or wheat sensitivity (NCWS) is a “clinical entity induced by the ingestion of wheat leading to intestinal and/or extraintestinal symptoms that improve once the wheat-containing foodstuff is removed from the diet, and celiac disease and wheat allergy have been excluded”. This mostly accepted definition raises several points that remain controversial on this condition. In the present review, the authors summarize the most recent advances in the clinic and research on NCWS through an accurate analysis of different studies. We screened PubMed, Medline, Embase, and Scopus using the keywords “non-celiac gluten sensitivity”, “non-celiac wheat sensitivity”, and “diagnosis”…

Carboxyl nonsteroidal anti-inflammatory drugs are efficiently glucuronidated by microsomes of the human gastrointestinal tract.

2004

Limited studies have been carried out on the biotransformation of carboxyl nonsteroidal anti-inflammatory drugs (NSAIDs) in the liver. However, the role of the intestine in NSAID metabolism has not been investigated. In this report, the contribution of UDP-glucuronosyltransferases (UGTs) in the human gastrointestinal (GI) tract from five donors to the glucuronidation of the NSAIDs, RS-ketoprofen, S-naproxen, RS- and S-etodolac, was investigated. UGT activity and, for some donors, mRNA levels were evaluated. All NSAIDs were glucuronidated throughout the GI tract; however, glucuronidation was low in stomach and duodenum as compared to the remainder of the intestine. RT-PCR analysis demonstrat…

Antagonism by SR 48692 of mechanical responses to neurotensin in rat intestine.

1996

Abstract 1. The effects of SR 48692 on neurotensin (NT)-induced mechanical responses were investigated in rat duodenum and proximal colon by use of isometric, isovolumic preparations. 2. SR 48692 inhibited the relaxant responses to NT in duodenal circular and longitudinal muscle. It also antagonized the NT-induced contractile effects in duodenal circular muscle and in proximal colon (both muscular layers). 3. From Schild analysis and pA2 value for SR 48692 was 8.2 in tissues where NT induced relaxant effects and 7.5 in tissues where NT induced contractile effects and the slope of the regression line was not significantly different from unity, indicating competitive antagonism. 4. SR 48692 d…

On the purinergic system in rat duodenum : existence of P1and P2receptors on the smooth muscle

1990

In rat duodenum, in vitro, in the presence of atropine and guanethidine, ATP administration caused a tetrodotoxin-insensitive relaxation followed by a rebound contraction. A similar response was obtained also after electrical field stimulation (EFS) of non-adrenergic, non-cholinergic (NANC) nerves. alpha, beta-methylene-TP and theophylline antagonized the response to ATP, but they failed to affect the noradrenaline- and EFS-induced relaxation. These results suggest that P1 and P2 receptors are present in rat duodenum, but their activation is not responsible for the inhibitor effects due to the NANC nerves.

Clinical Presentation of Crohn’s Disease

2015

Crohn’s disease is an idiopathic chronic inflammatory disease of the gut, which may involve the entire gastrointestinal tract from the mouth to the perianal area, though preferring in most cases the distal small bowel and the proximal large bowel. Its heterogeneous nature is reflected in a number of different phenotypes. Approximately 80 % of patients have small bowel involvement, usually in the distal ileum, with one-third of patients having exclusively ileitis. Approximately 50 % of patients have ileocolitis which refers to involvement of both the ileum and colon. From 20 to 25 % of patients have disease confined to the colon. Involvement of the esophagus, stomach, or duodenum is rare and…

Antihistaminic and anticholinergic activities of mequitazine in comparison with clemizole

1988

Abstract The antihistamine and anticholinergic properties of mequitazine have been investigated and compared with those of clemizole. Both mequitazine and clemizole antagonized the effect of histamine in guinea-pig ileum competitively, the pA2 values calculated by Schild plot were 9.95 ± 0.44 for mequitazine and 10.54 ± 0.44 for clemizole. Mequitazine at 10−7 M produced a parallel shift of the dose-response curve to acetylcholine in the rat duodenum, clemizole and the lower doses of mequitazine failed to modify the effect of acetylcholine. The potency of mequitazine and clemizole as H1-histamine blockers is similar, but only mequitazine at highest concentration used showed anticholinergic a…

Imaging Assessment of Gastroduodenal Perforations.

2016

Gastroduodenal perforation is an emergency situation that usually requires early recognition and well-timed surgical treatment. It can arise from different natural, iatrogenic, or traumatic causes, and it can present with various symptoms especially in the early phase. This article reviews the role of the different imaging techniques in the diagnosis of gastroduodenal perforation, focusing on the direct and indirect signs that are encountered in conventional radiography and computed tomography; our personal experience is also provided.
