Abstract Background: Intraoperative identification of nerve fibers heading from the inferior rectal plexus (IRP) to the internal anal sphincter (IAS) is challenging. The transanal total mesorectal excision (TaTME) is said to better preserve pelvic autonomic nerves. Transnasal endoscopy for direct visual control of esophageal stent placement without fluoroscopy

Placement of self-expanding metal stents (SEMSs) is a well-established treatment for esophageal stenosis and postoperative anastomotic leaks. Conventional endoscopic procedures for SEMS placement require fluoroscopic guidance, but transnasal endoscopy (TNE) with ultraslim endoscopes may allow precise stent release under direct visual control without the need for fluoroscopy. This prospectively collected data investigated the feasibility and safety of TNE-guided SEMS placement without fluoroscopy. Im Zeitraum Januar 1986 bis Juli 1991 wurden 393 Patienten endoskopisch operiert. Darunter waren 228 transanale endoskopische Operationen, 72 endoskopische Perforansdissektionen sowie 93 laparoskopische Cholezystektomien. Die Komplikationsrate fur transanale endoskopische Operationen kann mit 2,2%, fur die endoskopische Perforansvenendissektion mit 3% und fur die laparoskopische Cholezystektomie mit 7% angegeben werden. Die Rezidivrate lagen fur die transanale endoskopische Tumorabtragung bei 4% und fur die endoskopische Perforansvenendissektion bei 3%. Malignant and Unclear Histological Findings in Incidentalomas

<i>Background:</i> The management of incidentalomas with tumor size 3 cm and larger is still under controversial discussion. <i>Study Design:</i> Clinical charts of 65 patients who underwent adrenalectomy for an incidentaloma were reviewed. <i>Results:</i> Sixty-five patients were operated. There were 28 men and 37 women with a median age of 56.9 years. Median size of all resected lesions was 4.1 cm. Indications for surgery were tumor size equal and larger than 3 cm, recurrent pain, hormone status and patients’ fear of malignancy. In this technical note, we describe an innovative use of real-time mixed reality during robotic-assisted transanal total mesorectal excision. The endoscope is useful for the diagnosis and surgical therapy of diseases of the nose, the paranasal sinuses and its neighboring regions, and allows for microinvasive, functional approaches. The reduced invasiveness of therapeutic procedures is sometimes accompanied by insufficient clearness of the surgical field, however. This significant problem is solved by the computer-assisted-surgery (CAS) system, an intraoperative localizer.
