How to manage aspergillosis in non-neutropenic intensive care unit patients.

Invasive aspergillosis has been mainly reported among immunocompromised patients during prolonged periods of neutropenia. Recently, however, non-neutropenic patients in the ICU population have shown an increasing risk profile for aspergillosis. Associations with chronic obstructive pulmonary disease and corticosteroid therapy have been frequently documented in this cohort. Difficulties in achieving a timely diagnosis of aspergillosis in non-neutropenic patients is related to the non-specificity of symptoms and to lower yields with microbiological tests compared to neutropenic patients. Nonketotic hyperglycinemia (NKH) is an autosomal recessive inborn error in the glycine degradation pathway resulting in severe neurological impairment with intractable seizures and brain damage in the majority of the affected patients. Depending on the age of onset and on the outcome of the disease, severe and attenuated forms of NKH may be discriminated. During neonatal period, patients may present with early myoclonic encephalopathy; in the course of the disease, the picture of seizures changes, and multiple forms of seizures may occur. Backgroud The interpretation of thyroid function tests in the PICU patients can be difficult because thyroid hormone concentrations are affected by nonthyroidial stresses. Most patients fall into sick euthyroid syndrome or nonthyroidal illness syndrome (NTIS). The TH therapy during critical illness remains controversial. Aim To evaluate the NTIS in septic children. Percutaneous transrenal catheter retrieval.

Percutaneous transrenal catheter retrieval was successfully performed in 6 of 7 patients using the percutaneous transrenal access. Three broken nephrostomy tubes and 3 indwelling splints were extracted. The instruments used were deflecting guide wire in 1, grasping forceps in 2, and Dormia stone basket in 3 instances. No complications were encountered.

Ergebnisse und Komplikationen von 616 perkutanen transhepatischen Gallenwegsdrainagen

During nine years, percutaneous transhepatic biliary drainage was carried out 616 times on 563 patients in the Department of Radiology, University of Mainz Medical School. 50.3% were pre-operative and 39% were palliative. More than 80% were necessitated by malignant lesions. Subsequent improvements in biochemical measurements were observed in 82.4% of patients. Complications of the procedure led to the death of five patients (0.8%) and required surgery in nine patients (1.5%). The following complications were observed: biliary peritonitis in 0.6%, sepsis in 1.9%, bleeding in 1.9% and fever higher than 38 degrees C in 16.2%.

32 forceps biopsies were performed in 30 patients with obstructive jaundice during percutaneous transhepatic biliary drainage procedures. In one patient an adequate specimen could not be obtained. In 25 of the remaining 31 cases diagnosis was confirmed histologically (malignant tumours: n = 22, benign stricture: n = 3). In 6 patients false-negative results were obtained. Transluminal biopsy is an easily performed adjunct to percutaneous transhepatic diagnostic interventions with minimal additional discomfort for the patient. In many cases percutaneous needle biopsy can be avoided. Forceps biopsy enables nonoperative histological diagnosis of small carcinomas of the bile ducts.

Conditioning effect of transcranial magnetic stimulation evoking motor‐evoked potential on V‐wave response

Abstract The aim of this study was to examine the collision responsible for the volitional V‐wave evoked by supramaximal electrical stimulation of the motor nerve during voluntary contraction. V‐wave was conditioned by transcranial magnetic stimulation (TMS) over the motor cortex at several inter‐stimuli intervals (ISI) during weak voluntary plantar flexions (n = 10) and at rest for flexor carpi radialis muscle (FCR; n = 6). Knee extension strength and walking speed in relation to quadriceps muscle composition and training in elderly women.

Summary. Knee extension strength, walking speed, quadriceps muscle mass and composition of the muscle compartment were studied in 66 to 85-year-old female athletes and controls. Maximal voluntary knee extension force, force/body mass, extension torque, torque/body mass and walking speed were higher for the athletes than the controls. A muscle index indicating intramuscular fat and connective tissue measured using ultrasonography was lower for the athletes than the controls.
