Evaluation of surgical decision making and resulting outcome in patients with highly eloquent glioblastoma: Results of a multicenter assessment.

2017

Treatment of glioblastoma(GB) patients amenable only for a subtotal resection(STR) is controversial. Since outcome of patients is affected by surgical management, our aim was to assess surgical decision making and resulting outcome in patients with highly eloquent GBs.We retrospectively assessed GB patients with intended sub-total resection (STR) or stereotactic biopsy (STX) of 3 neurooncological centers operated between 2008 and 2013. A volumetric assessment of overall extent of resection(oEoR), presence of complications, new permanent neurological deficits(nPNDs) was performed. A central reviewer reassessed all cases blinded and gave recommendation on surgical management and on a potentia…

New prognostic markers in neuroblastoma

2013

Introduction: The hallmark of neuroblastoma is its clinical and biological heterogeneity, with the likelihood of cure varying widely according to age at diagnosis, extent of disease and tumor biology. We hope this review will be useful for understanding part of the unfamiliar neuroblastoma codex. Areas covered: In the first part of this review, the authors summarize the currently used prognostic factors for risk-adapted therapy, with the focus on clinical management of neuroblastoma patients. In the second part, the authors discuss the evolving prognostic factors for future treatment schemes. A search of online medical research databases was undertaken focusing especially on literature publ…

More is more? : Forest management allocation at different spatial scales to mitigate conflicts between ecosystem services

2017

Context: Multi-objective management can mitigate conflicts among land-use objectives. However, the effectiveness of a multi-objective solution depends on the spatial scale at which land-use is optimized. This is because the ecological variation within the planning region influences the potential for site-specific prioritization according to the different objectives. Objectives: We optimized the allocation of forest management strategies to maximize the joint production of two conflicting objectives, timber production and carbon storage, at increasing spatial scales. We examined the impacts of the extent of the planning region on the severity of the conflict, the potential for its mitigation…

Distance decay 2.0 – a global synthesis of taxonomic and functional turnover in ecological communities

2021

AbstractUnderstanding the variation in community composition and species abundances, i.e., β-diversity, is at the heart of community ecology. A common approach to examine β-diversity is to evaluate directional turnover in community composition by measuring the decay in the similarity among pairs of communities along spatial or environmental distances. We provide the first global synthesis of taxonomic and functional distance decay along spatial and environmental distance by analysing 149 datasets comprising different types of organisms and environments. We modelled an exponential distance decay for each dataset using generalized linear models and extracted r2 and slope to analyse the streng…

Development of artificial neural network for condition assessment of bridges based on hybrid decision making method – Feasibility study

2021

Abstract Managing a bridge at an appropriate level of reliability requires knowledge of its technical condition, which is decisive in terms of maintenance and repair activities. This is a multi-criteria decision-making problem which results from the need to allocate limited financial resources to this work. Although many calculation models have been suggested in published sources, none of them has ever met these requirements. The algorithm presented by the authors allows for the assessment of any number of bridges, taking into account the diversity of solutions in terms of materials and structures, and can provide a solution to this problem. This hybrid calculation model, combining the modi…

Extent of resection and survival in glioblastoma multiforme

2016

Brown and colleagues1 reported the results of a systematic review of the literature aimed in determining whether greater extent of resection (EOR) is associated with improved 1- and 2-year overall survival and 6-month and 1-year progression-free survival in patients affected by glioblastoma multiforme. The analysis revealed 37 studies suitable for inclusion. The authors found that gross total resection (GTR) for glioblastoma multiforme reduces 1- and 2-year mortality, thus supporting GTR over subtotal resection and biopsy.

With a Little Help from My Friends: The Role of Intraoperative Fluorescent Dyes in the Surgical Management of High-Grade Gliomas

2018

High-grade gliomas (HGGs) are the most frequent primary malignant brain tumors in adults, which lead to death within two years of diagnosis. Maximal safe resection of malignant gliomas as the first step of multimodal therapy is an accepted goal in malignant glioma surgery. Gross total resection has an important role in improving overall survival (OS) and progression-free survival (PFS), but identification of tumor borders is particularly difficult in HGGS. For this reason, imaging adjuncts, such as 5-aminolevulinic acid (5-ALA) or fluorescein sodium (FS) have been proposed as superior strategies for better defining the limits of surgical resection for HGG. 5-aminolevulinic acid (5-ALA) is i…

Integration of local features into a global shape

2001

AbstractIt is a well-established fact that the cortical filters selective for spatial frequency and orientation have a limited spatial extent. The present study investigated how information from local filters is integrated into global shapes. Specifically, we were interested in whether identification of a global pattern consisting of small oriented, spatially-bandpass features (Gabor patches) depends on the orientations of those features. The observer was presented with a C-like stimulus shape comprised of oriented Gabor elements on a blank background, and the performance measure was the threshold contrast for identifying the global orientation of the C-shape (four possible rotated orientat…

Maximizing the Extent of Resection in High-Grade Glioma.

2019

High-grade gliomas (HGGs) are devastating tumors associated with one of the worst prognoses in oncology. Glioblastoma multiforme (GBM) is the most frequently reported histologic type, with a median survival after surgery and combined treatment with chemotherapy and radiotherapy of 12–16 months. Several studies have shown that the extent of resection (EOR) of the contrast-enhancing part of the tumor improves survival in patients with HGGs, although the quest to achieve optimal oncologic outcomes must be tempered with the neurologic result following radical resection. To date, limited evidence exists on the relationship between EOR and level of clinical benefit for patients with HGGs. Brown e…
