D-penicillamine in Wilson's disease presenting as acute liver failure with hemolysis.

Wilson's disease in a young woman presenting with an acute course is described. The clinical manifestations were fulminant hepatic failure associated with marked intravascular hemolysis. Immediate D-penicillamine and high-dose steroid therapy did not influence the course of the disease. Necropsy revealed an increased hepatic copper content and cirrhosis with extensive necrosis of the liver.

PaO2oscillations caused by cyclic alveolar recruitment can be monitored in pig buccal mucosa microcirculation

BACKGROUND Cyclic alveolar recruitment and derecruitment play a role in the pathomechanism of acute lung injury and may lead to arterial partial pressure of oxygen (PaO(2) ) oscillations within the respiratory cycle. It remains unknown, however, if these PaO(2) oscillations are transmitted to the microcirculation. The present study investigates if PaO(2) oscillations can be detected in the pig buccal mucosa microcirculation. METHODS Respiratory failure was induced by surfactant depletion in seven pigs. Tumor necrosis factor alpha (TNF-alpha) antagonists have emerged as an effective therapy for patients with diseases as Crohn's disease, rheumatoid arthritis, and other chronic systemic inflammatory diseases. In the last years, there has been a growing interest in the role that inflammatory cytokines, which sustain the pathogenesis of these diseases, plays in regulating cardiac structure and function, particularly in the progression of chronic heart failure. Increased oxidative stress has been implicated in the pathogenesis of a number of cardiovascular diseases. Recent findings suggest that myeloperoxidase (MPO) may play a key role in the initiation and maintenance of chronic heart failure (CHF) by contributing to the depletion of the intracellular reservoir of nitric oxide (NO). This review examines the most important aspects of the use of topical corticosteroids in oral pathology. We provide a detailed analysis of the most common types of corticosteroids used, their methods of application and the clinical data that must be managed in order to monitor the effectiveness of the treatment. We also analyze the correctable causes of treatment failure and the possible adverse effects that may occur after applying these drugs. All of this is achieved by conducting a critical review of these concepts.

Exercise-associated hyponatremia (EAH) is dilutional hyponatremia, a variant of inappropriate antidiuretic hormone secretion (SIADH), characterized by a plasma concentration of sodium lower than 135 mEq/L. The prevalence of EAH is common in endurance (<6 hours) and ultra-endurance events (>6 hours in duration), in which both athletes and medical providers need to be aware of risk factors, symptom presentation, and management. The development of EAH is a combination of excessive water intake, inadequate suppression of the secretion of the antidiuretic hormone (ADH) (due to non osmotic stimuli), long race duration, and very high or very low ambient temperatures. Abstract Background. Liver transplantation is the treatment of choice for end-stage liver disease in both adult and paediatric patients. The Italian experience in paediatric liver transplantation during the period 1988–1999 is reported herein. Patients and methods. This report concerns 228 liver transplantations performed in 207 patients (100 male, 107 female, mean age 5.1±4.4 years) in 11 Italian centres. Octaplex in routine clinical use for prophylaxis and therapy of bleeding in patients with prothrombin complex factor deficiency

Octaplex is a new prothrombin complex concentrate that is indicated for treatment or perioperative prophylaxis of bleeding in patients with deficiency of the prothrombin complex coagulation factors, such as deficiency caused by treatment with vitamin K antagonists or by liver failure, when rapid correction of bleeding is required. The study was conducted to demonstrate both prevention of bleeding and achievement of haemostasis in acute bleeding and to obtain further information about the safety of administration of Octaplex.
