An 1076 chirurgischen und internistischen Abteilung in der BRD (West) wurde eine Umfrage zur Behandlung der Osophagusvarizenblutung durchgefuhrt. Es uberwiegt die Akutsklerosierung bzw. die Ballonsondenbehandlung mit nachfolgender Sklerosierung bei der aktiven Blutung. Die abdominelle Sperroperation wird bei der konservativ nicht stillbaren Blutung noch vor dem portokavalen Notshunt bevorzugt. Betablocker werden vor allem in internistischen Abteilungen zur Rezidivprophylaxe eingesetzt. Erst nach mehrfacher Rezidivblutung trotz Sklerosierung sehen etwa die Halfte der Abteilungen eine Indikation zum elektiven Shunt. Adjustable tourniquet to manipulate pulmonary blood flow after Norwood operations

Abstract Background . Survival after first-stage palliative Norwood operations for single ventricle with systemic outflow obstruction is mainly dependent on a balanced ratio of pulmonary blood flow to systemic blood flow. Here we report the clinical results using a modified technique that allows a controlled systemic-to-pulmonary shunt flow to prevent pulmonary overcirculation. Methods . From 1995 to 1998, of 26 infants undergoing first-stage palliative Norwood operations, 7 had placement of an adjustable tourniquet around a modified right Blalock-Taussig shunt. Results . Portal hypertension is defined by an increase in the portosystemic venous gradient. In most cases, increased resistance to portal blood flow is the initial cause of elevated portal pressure. More than 90% of cases of portal hypertension are estimated to be due to advanced chronic liver disease or cirrhosis. Transjugular intrahepatic portosystemic shunts, a non-pharmacological treatment for portal hypertension, involve the placement of a stent between the portal vein and the hepatic vein or inferior vena cava which helps bypass hepatic resistance. In a coastal salt-marsh of Spain, the digger wasp Stizus continuus primarily hunted for grasshoppers of the genus Heteracris, revealing (at least in the period of the study and in this area) this wasp to be almost monophagous. In contrast, grasshoppers of the genus Acrotylus were ignored by the wasps in spite of their high abundance in the environment. We hypothesize that this bias occurred because Acrotylus is found more often on the soil and on grasses, while Heteracris is nearly only found on Sarcocornia bushes, which probably represent the habitat mostly exploited by the wasps for hunting. Optineurin is present in protein inclusions observed in various neurodegenerative diseases including amyotrophic lateral sclerosis (ALS), Huntington's disease, Alzheimer's disease, Parkinson's disease, Creutzfeld-Jacob disease and Pick's disease. Huntington's disease (HD) is a hereditary neurodegenerative disease caused by the expansion of a polyglutamine stretch in the huntingtin (HTT) protein and characterized by dysregulated calcium homeostasis. We investigated whether these disturbances are correlated with changes in the mRNA level of the genes that encode proteins involved in calcium homeostasis and signaling (i.e., the calciosome). Using custom-made TaqMan low-density arrays containing probes for 96 genes, we quantified mRNA in the striatum in YAC128 mice, a model of HD, and wildtype mice. AAV Vector–mediated RNAi of Mutant Huntingtin Expression Is Neuroprotective in a Novel Genetic Rat Model of Huntington's Disease

We report the characterization of a new rapid-onset model of Huntington's disease (HD) generated by adeno-associated virus (AAV) vector–mediated gene transfer of N-terminal huntingtin (htt) constructs into the rat striatum. Expression of exon 1 of mutant htt containing 70 CAG repeats rapidly led to neuropathological features associated with HD. In addition, we report novel data relating to neuronal transduction of AAV vectors that modulated the phenotype observed in this model. Quantitative reverse transcriptase–polymerase chain reaction (RT–PCR) revealed that AAV vector–mediated expression in the striatum increased by >100-fold as compared to the endogenous htt level.
