Le repérage des patients en situation de précarité par les médecins généralistes

Cette étude se propose d’étudier la concordance entre un repérage de la précarité effectué spontanément par les médecins généralistes et un repérage effectué par le score EPICES (évaluation de la précarité et des inégalités de santé dans les centres d’examens de santé), développé et validé dans les centres d’examens de santé.Les 32 médecins généralistes ont permis d’inclure 207 patients dans l’étude. Le coefficient de concordance Kappa de Cohen était de 0,404, soit une corrélation faible à modérée.L’étude des variables associées à la concordance de ce repérage n’a permis de dégager aucune variable significativement associée à la concordance après une analyse multivariée.Pour conclure, le re…

Modeling of medical activity heterogenity to improve the hospital management by diagnosis-related groups

Since the early 1980s healthcare systems in the industrialized nations have been undergoing radical reform aimed at curbing overspending of hospital expenditure. After a discussion of the limits of a prospective payment due to heterogeneity of costs within DRGs, we demonstrate the ability of anovel statistical model to identify high cost patients. We derive from this statistical model an economic heuristic in order to account for high cost patients in budget allocation and a Structural and Contingent method is proposed as a budgeting tool. The lyon clinical olfactory test: validation and measurement of hyposmia and anosmia in healthy and diseased populations.

 ; International audience; The LCOT is a self-administered test designed to assess olfactory deficits. Altogether, 525 subjects contributed to the validation. Elderly participants were well represented in this sample. In a validation study (study 1), 407 healthy and 17 anosmic volunteers between 15 and 91 years of age underwent threshold, supraliminal detection, and identification testing. Cutoff values for normosmia and hyposmia were calculated and applied in a second study in a group of patients with smell complaints and in a group of Alzheimer patients with age-matched controls.
