Idiopathic trigeminal sensory neuropathy with gadolinium enhancement in the cisternal segment

The authors report two patients with idiopathic trigeminal sensory neuropathy who showed gadolinium enhancement of the cisternal segment of the corresponding trigeminal nerve in cranial MRI. The resolution of these lesions in a repeat MRI suggests a similarity to Bell's palsy.

La diabetes mellitus (DM) es un problema creciente de salud pública en todo el mundo. En España la mayoría de estudios muestran ratios de prevalencia entre el 10% y el 15%. Los estudios de prevalencia de polineuropatía diabética (PNPD) muestran resultados muy dispares que oscilan entre un 22,7% a un 54%. Un 70% de las amputaciones de miembros inferiores suceden en personas con DM y un 85% de todas las amputaciones están precedidas por una úlcera. Se considera que el 25% de los pacientes con DM desarrollará úlcera plantar y un 2% requerirá amputación. Objective :To assess the function of the optic pathway in type 2 diabetics without retinopathy (40 subjects) or with mild Non Proliferative Diabetic Retinopathy (40 subjects). Methods: VEPs , elicited by means of pattern-reversal stimulation, were studied in a group of 80 non-insulin-dependent diabetics . Results: In subjects suffering from diabetes for less than 10 years, with mild retinopathy or without retinopathy, the amplitude N75-P100 decreased, and this is statistically significant with stimulation at 15’. Neuropathic pain is defined as pain initiated or caused by a primary lesion or dysfunction in the nervous system (Merskey and Bogduk, 1994). Current efforts to refine this definition focus on the terms ‘dysfunction’ and ‘nervous system’ with the intention to clarify that there has to be an identifiable lesion or disease process affecting the somatosensory system. Abstract For the diagnosis of anti-MAG polyneuropathy the commercial ELISA manufacturer currently recommends a cut-off of 1000 Bühlmann Titer Units (BTU). We analyzed sera from 80 anti-MAG neuropathy patients and 383 controls (with other neuropathies or healthy controls) to assess the ELISA sensitivity and specificity at different thresholds.
