Altered Regulation of Insulin-Like Growth Factor Binding Protein-I in Patients With Polycystic Ovary Syndrome
1995
Insulin-like growth factor (IGF-I) action is influenced by circulating as well as tissue levels of its binding proteins. Because serum IGF binding protein-1 (IGFBP-1) levels have been found to be decreased in patients with polycystic ovary syndrome (PCOS), we tested the hypothesis that regulation of IGFBP-1 secretion may be different in patients with PCOS compared with normal women. We studied 15 normal ovulatory women and 15 women with PCOS of similar age (21 ± 1 and 22 ± 1 years, respectively). All subjects were studied after an overnight fast between days 5–8 after spontaneous or progestin-induced menses. Perturbations included the administration of insulin intravenously, maintenance of …
The Mediterranean diet: A historical perspective on food for health.
2020
The Mediterranean diet (MedDiet) represents the crystallisation of the centuries-old cooking legacies of different civilisations. The association of the MedDiet with longevity and low cardiovascular risk prompted the Seven Countries Study, which provided epidemiological evidence on the health effects of diet. This led to further studies, both epidemiological and interventional. Scales to measure adherence as well as studies of food components have consolidated a body of knowledge that is of great interest to institutions and governmental agencies. The recognition of its benefits for health has made the widespread introduction of the MedDiet an urgent challenge. Parallel efforts are being ma…
Three-dimensional/four-dimensional transperineal ultrasound: clinical utility and future prospects.
2017
During the last decade, there has been a huge advancement in the use of transperineal ultrasound (TPU) in the field of obstetrics and gynecology. Its main applications in obstetrics include the monitoring of fetal progression in labor and recently the assessment of maternal pelvic dimensions, whereas in gynecology, TPU is at present widely used for the evaluation of the female pelvic floor, opening new boundaries for the assessment of pelvic floor disorders. The association of volumetric three-dimensional techniques has largely contributed to the remarkable progress that has occurred in the use of TPU. The aim of this paper is to offer an overview of the advantages, challenges and future pe…
Intramuscular Abdominal Wall Endometriosis, an Unusual Finding for General Surgeon
2021
Abstract Defined as the ectopic development of uterine tissue outside the uterine cavity, endometriosis is an increasingly common condition that can lead to various complications from chronic pain syndrome, infertility, obstruction due extrinsic compression to malignancy of endometriosis foci. Extrapelvic positioning of endometriosis is rare, diagnosis can be difficult both clinically and imaging, and treatment does not always ensure the absence of recurrences.
Effectiveness and quality of life 10 years after transobturator suburethral tape surgery for stress urinary incontinence
2018
AIM We aimed to measure the effectiveness and quality of life (QoL) 10 years after transobturator suburethral tape surgery. METHODS We carried out a prospective observational study of 42 women assessed 10 years after surgical intervention with the transobturator technique. The main outcome measures were subjective and objective cure or improvement, complications, and changes in QoL. RESULTS The median abdominal leak point pressure had been 92 (82-113) mL H2 O prior to surgery. Thirty-six women (85.7%) remained cured (negative cough test) and 13 (31%) reported urine leakage during physical activity, percentages which were similar to those at a previous assessment at the 5th year. Urgency uri…

2021
Abstract Objective To provide national guidelines for the management of women with severe pre-eclampsia. Design A consensus committee of 26 experts was formed. A formal conflict-of-interest (COI) policy was developed at the onset of the process and enforced throughout. The entire guidelines process was conducted independently of any industrial funding. The authors were advised to follow the principles of the Grading of Recommendations Assessment, Development and Evaluation (GRADE(r)) system to guide assessment of quality of evidence. The potential drawbacks of making strong recommendations in the presence of low-quality evidence were emphasised. Methods The last SFAR and CNGOF guidelines on…
Modern Aspects in the Diagnosis and Surgical Treatment of Breast Cancer in Early Stages
2021
Abstract Breast cancer is the most common cancer in women and a major public health problem, affecting 2.1 million women each year globally and causing the highest number of cancer-related deaths among women. In 2018, an estimated 627,000 women died from breast cancer (about 15% of all cancer deaths among women). To improve survival, early detection is essential. There are two strategies for early detection of breast cancer: early diagnosis and screening.(1) In Romania, the latest statistics show a higher incidence of breast cancer compared to the European average of 7929 new cases / year with a mortality of 3101 deaths / year.(2)
Association of first trimester fetal heart rate and nuchal translucency with preterm birth.
2021
To evaluate whether first trimester fetal heart rate (FHR) and nuchal translucency (NT) associate with preterm birth (PTB).This was a comparative case-control study of 518 normal pregnancies with no history of PTB, of which 272 delivered at term (TB) and 246 progressed to spontaneous PTB prior to 37, 34, 32, and 28 weeks. Fetal heart rate (FHR) and NT values at the first-trimester scan were compared by means of univariable (Mann-Whitney) and multivariable logistic regression analysis considering hourglass membranes (HM) as the most severe PTB subgroup. Finally, severity trends for both parameters were investigated using correlations with gestational age (GA) at delivery and Kruskal-Walls te…
How to understand the complexity of endometriosis-related pain
2017
Pain is the most important symptom in patients with endometriosis, and its management is truly challenging. Due to the different localization of the endometriotic lesions in the pelvis, patients suffer from visceral and somatic pain or both at the same time. There are specific and unspecific symptoms characterized by endometriosis. Specific symptoms include dysmenorrhea, cyclic and acyclic pelvic pain, dyschezia, dysuria and dyspareunia. There is also a wide range of unspecific symptoms, such as unspecific bowel and bladder complaints, the emission of pain in the legs, vegetative concomitants like vomiting, emesis, gastric disorders, headaches, dizziness, painful ovulation, irregular pelvic…
Invasive ovarian seromucinous carcinoma developed on an endometrioma. The determining role of transvaginal ultrasound
2021
Endometriosis is a chronic inflammatory disease that has been associated with an increased risk of ovarian cancer, especially epithelial tumors. Transvaginal ultrasound is a first-line diagnostic method for the assessment of endometriosis, and of endometriomas, the ovarian cystic form of endometriosis. Endometriomas usually have typical ultrasound features that allow them to be easily diagnosed. When atypical images are observed, such as solid areas with vascular flow, their malignancy should be ruled out, especially in peri-menopausal women. Early diagnosis of these tumors is essential because immediate surgical treatment is crucial for prognosis. Malignant neoplasms associated with endome…
