Se ha estimado que la pancreatitis aguda (PA) tiene una mortalidad en torno al 5%, siendo practicamente nula en las PA leves y llegando al 30%-50% en las PA graves. Se ha determinado que el fallo orgánico (FO) y la infección de la necrosis pancreática son los dos principales determinantes de la mortalidad. Se ha observado que el FO que se resuelve en 48 horas o menos (FO transitorio) tiene mejor pronóstico que el FO que se prolonga durante más de 48 horas (FO persistente). No existen estudios que valoren la importancia del FO de inicio (FOI) que se desarrolla en las primeras 48 horas tras el ingreso. Objectives To assess the evidence for tobacco smoking as a risk factor for the causation of chronic pancreatitis. Methods We performed a meta-analysis with random-effects models to estimate pooled relative risks (RRs) of chronic pancreatitis for current, former, and ever smokers, in comparison to never smokers. We also performed dose-response, heterogeneity, publication bias, and sensitivity analyses. Results Ten case-control studies and 2 cohort studies that evaluated, overall, 1705 patients with chronic pancreatitis satisfied the inclusion criteria. After early operation in 49 patients and delayed operation in 114 patients, all with acute hemorrhagic-necrotizing pancreatitis, 65% of patients developed local or general complications. Local complications were abscesses, peritonitis, bleeding, gastrointestinal fistulae or stenoses, and external pancreatic fistulae. Their cause can be traced to the large wound cavity with the tryptic wound surface as well as residual necrosis. The general postoperative complications were shock, acute renal failure, cardiorespiratory insufficiency, gastrointestinal bleeding, ileus, coagulopathy, and sepsis. Hemorrhage from the pancreatic duct, i.e. hemosuccus pancreaticus (HP), is a rare cause of gastrointestinal bleeding. Pancreatic hemosuccus is usually due to the rupture of an aneurysm of a visceral artery, most likely the splenic artery, in chronic pancreatitis. Other causes of HP are rare.
