Background - Direct acting antivirals (DAAs) for hepatitis C virus (HCV) have quite changed the therapeutic routine in the last few years. Reported rates of sustained virological response (SVR), exceed 90% in all patients subgroups. Other, even more effective, DAAs combinations are being developed for use in the most difficult to treat and advanced patients, aiming for 100% of SVR in 100% of cases. The results of scientific research confirm that the condition of insularity is an issue that still needs broad sharing and specific policies. The issues to be addressed are widespread in all territorial contexts, large or small. Insularity is a permanent structural constraint. Even though convergence policies have contributed to the convergence process between countries and regions, EU policies for the islands must be envisaged to define joint action to achieve a more balanced territorial equity.
