Goitrous congenital hypothyroidism in a twin pregnancy causing respiratory obstruction at birth: Implications for management

We report a twin pregnancy complicated by fetal goitrous hypothyroidism secondary to dyshormonogenesis caused by thyroglobulin deficiency. Antenatal treatment with intra-amniotic thyroxine was considered but not performed, given the late gestational age at diagnosis and the multiple nature of the pregnancy. Both twins developed airway obstruction at delivery, requiring intubation and ventilation. We review the literature and describe the practical issues relating to the antenatal assessment and perinatal management of fetal goitre.

Recent epidemiological studies on ionizing radiation and childhood cancer in Germany.

The results from recent epidemiological studies based on the German Childhood Cancer Registry are summarized.An oecological study covers 16 years' incidence of childhood malignancies in areas surrounding nuclear facilities and in matched control regions. Two population-based case control studies explore potential risk factors of childhood leukaemia in the state of Lower Saxony (1988 93) and in all western states of Germany (1992-94).There was no increased incidence of childhood leukaemia and other malignancies in the vicinity of nuclear installations. Prenatal diagnosis and epidemiology of multicystic kidney dysplasia in Europe

ObjectivesThe aim of this study is to describe the prenatal diagnosis and epidemiology of multicystic kidney dysplasia (MCKD). MethodsThe study is based on routinely collected data from a European database of major congenital anomalies including 13 registries with cases born in 1997-2006 and covering 1458552 births. ResultsThere were 601 MCKD cases giving an overall prevalence of 4.12 per 10000 births with regional variation. In live births, 87% of cases had an isolated renal anomaly and 13% had associated major nonrenal anomalies (chromosomal, syndrome or other major anomalies). Long-term prediction of birth weight

On the basis of the hypothesis that undisturbed individual growth in fetal life keeps a constant proportional difference with the standard population 50th percentile, birth weight can be predicted with a single sonographic exploration after the 16th week of pregnancy. Data on 135 singleton pregnancies with accurate dates and delivery at term were used. Sonography was performed between the 17th and 36th weeks of pregnancy, in every case at least 4 weeks before delivery. Ovarian cysts in the fetus and neonate--changes in sonographic pattern in the follow-up and their management.

In a multicenter trial we retrospectively evaluated the clinical and sonographic data of 49 neonatal ovarian cysts, 44 of which were detected prenatally and 5 on the first day after delivery. Of the 44 prenatally detected cysts 39 were purely cystic, 5 echogenic or had a mixed pattern. In 20 patients the cystic appearance changed during delivery from purely cystic to a mixed pattern being independent on the size of the cyst. 26 of the 44 cysts were treated surgically. Salpingotorsion was found in 8 and was independent on the size of the cyst. Treatment of congenital abdominal wall defects —a 25-year review of 132 patients

The outcome data of 132 patients treated at the Department of Pediatric Surgery in Mainz during the last 25 years were reviewed. Prenatal diagnosis of abdominal wall defects (AWDs) and associated malformations led to increasing selection of the patient population. The aim of primary closure of the abdominal wall can be achieved more frequently in gastroschisis (GS) than omphalocele (OC), while the postoperative course is more complicated and of longer duration in GS. Delayed or secondary closure extended the hospitalization period but had no negative effects on the outcome. Thyroid hormones induce sumoylation of the cold shock domain-containing protein PIPPin in developing rat brain and in cultured neurons.

We previously identified a cold shock domain (CSD)-containing protein (PIPPin), expressed at high level in brain cells. PIPPin has the potential to undergo different post-translational modifications and might be a good candidate to regulate the synthesis of specific proteins in response to extracellular stimuli. Here we report the effects of thyroid hormone (T3) on PIPPin expression in developing rat brain. We found that a significant difference among euthyroid- and hypothyroid- newborn rats concerns sumoylation of nuclear PIPPin, that is abolished by hypothyroidism.
