The Use of Neoadjuvant Chemotherapy in Patients With Urothelial Carcinoma of the Bladder: Current Practice Among Clinicians.

2017

Abstract Introduction Guidelines recommend neoadjuvant chemotherapy (NAC) before radical cystectomy (RC) in patients with urothelial carcinoma of the bladder in clinical stages T2-T4a, cN0M0. We examined the frequency and current practice of NAC and sought to identify predictors for the use of NAC in a prospective contemporary cohort. Materials and Methods We analyzed prospective data from 679 patients in the PROMETRICS (PROspective MulticEnTer RadIcal Cystectomy Series 2011) database. All patients underwent RC in 2011. Uni- and multivariable regression analyses identified predictors of NAC application. Furthermore, a questionnaire was used to evaluate the practice patterns of NAC at the PR…

Coincident vesical transitional cell carcinoma and prostatic carcinoma. Clinical features and treatment.

1986

Summary— Twenty-two patients with vesical urothelial carcinoma associated with prostatic carcinoma were reviewed. They represented 1.5% of the bladder and prostatic tumours treated in our department within a 12-year period from 1968 to 1979. Their management included several treatment policies, based on the separate assessment of each tumour variant. For non-infiltrating bladder tumours, transurethral tumour resection was combined with hormonal treatment, external radiotherapy or resection of the prostate depending on the stage of the prostatic tumour. Radical cystoprostatectomy was performed for two cases of infiltrating bladder tumour with well localised prostatic tumours. A conservative …

Radical cystectomy or bladder preservation with radiochemotherapy in elderly patients with muscle-invasive bladder cancer: Retrospective Internationa…

2017

IF 3.156; International audience; Background: Radical cystectomy (RC) and radiochemotherapy (RCT) are curative options for muscle-invasive bladder cancer (MIBC). Optimal treatment strategy remains unclear in elderly patients.Material and methods: Patients aged 80 years old and above with T2-T4aN0-2M0-Mx MIBC were identified in the Retrospective International Study of Cancers of the Urothelial Tract (RISC) database. Patients treated with RC were compared with those treated with RCT. The impact of surgery on overall survival (OS) was assessed using a Cox proportional hazard model. Progression included locoregional and metastatic relapse and was considered a time-dependent variable. Results: Be…

The efficacy of BCG TICE and BCG Connaught in a cohort of 2,099 patients with T1G3 non–muscle-invasive bladder cancer

2016

BACKGROUND Potential differences in efficacy of different bacillus Calmette-Guerin (BCG) strains are of importance for daily practice, especially in the era of BCG shortage. OBJECTIVE To retrospectively compare the outcome with BCG Connaught and BCG TICE in a large study cohort of pT1 high-grade non-muscle-invasive bladder cancer patients. DESIGN, SETTING, AND PARTICIPANTS Individual patient data were collected for 2,451 patients with primary T1G3 tumors from 23 centers who were treated with BCG for the first time between 1990 and 2011. OUTCOME MEASUREMENTS AND STATISTICAL ANALYSIS Using Cox multivariable regression and adjusting for the most important prognostic factors in this nonrandomiz…

Postoperative nomogram for invasive bladder cancer: does it really work? A multicenter cohort study.

2011

Objectives: Nomograms are statistical models designed to maximize predictive accuracy. We have tested the statistical correlation between the predictions of International Bladder Cancer Nomogram Consortium and the clinical outcomes in a multicenter Italian cohort of patients treated with radical cystectomy (RC) and pelvic lymph-nodes dissection. Methods and materials: Two hundred four patients who underwent RC were selected for multiple variable and then enrolled in the study. Patients were tested by the "online tool" based on the nomogram, then stratified and risk grouped for 5-year predicted disease-free survival (pDFS): low risk (67%-100%), intermediate risk (34%-66%), and high risk grou…

An increased body mass index is associated with a worse prognosis in patients administered BCG immunotherapy for T1 bladder cancer

2018

PURPOSE: The body mass index (BMI) may be associated with an increased incidence and aggressiveness of urological cancers. In this study, we aimed to evaluate the impact of the BMI on survival in patients with T1G3 non-muscle-invasive bladder cancer (NMIBC). METHODS: A total of 1155 T1G3 NMIBC patients from 13 academic institutions were retrospectively reviewed and patients administered adjuvant intravesical Bacillus Calmette-Guérin (BCG) immunotherapy with maintenance were included. Multivariable Cox regression analysis was performed to identify factors predictive of recurrence and progression. RESULTS: After re-TURBT, 288 patients (27.53%) showed residual high-grade NMIBC, while 867 (82.8…

Effects of a small seagull colony on trophic status and primary production in a Mediterranean coastal system (Marinello ponds, Italy)

2012

Abstract Colonies of seabirds have been shown to influence nutrient cycling and primary production of coastal areas, but knowledge is still limited above all for smaller colonies. This study evaluates the influence of a small resident seagull colony (Larus michahellis Naumann, 1840) on a Mediterranean coastal system (Marinello ponds, Sicily, Italy). The presence of ornithogenic organic matter from seagull guano was first assessed at increasing distances from the colony using δ15N to indicate the effects of guano on the trophic status and primary production. The pond directly affected by guano deposition showed an anomalous water and sediment chemistry, especially regarding physico-chemical …

Spatial and temporal changes of suspended matter in relation to wind and vegetation cover in a mediterranean shallow coastal environment

1999

Seasonal and spatial changes in seston, (POC), particulate organic carbon, (PON) particulate organic nitrogen and chlorophyll-a concentrations were studied on a monthly basis in a Mediterranean shallow coastal area (Stagnone di Marsala, Western Sicily) in order to gather information on factors controlling particulate organic matter distribution and composition. Seston concentration and composition were connected to the main physicochemical and biological driving factors, such as temperature, salinity, dissolved oxygen, wind-speed and biomass of submerged vegetation. The Stagnone di Marsala is characterized by high temperatures with strong seasonality (range: 11-28°C), while values ranged fr…
