High/low-volume center experience predicts outcome of AMS 800 in male stress incontinence: Results of a large middle European multicenter case series.

AIM To analyze the influence of implantation volume of artificial sphincters (AMS 800) on outcome in a large central European multicenter cohort study. METHODS As part of the DOMINO (Debates on Male Incontinence) project, the surgical procedures and outcomes were retrospectively analyzed in a total of 473 patients who received an artificial sphincter (AMS 800) between 2010 and 2012. Clinics that implanted at least 10 AMS 800 per year were defined as high-volume centers. RESULTS Sixteen centers had a mean rate of 9.54 AMS 800/y of which five clinics were identified as high-volume centers. In humans, the actions of angiotensin II are transduced through the AT1 and AT2 receptors which have recently been implicated in renal organogenesis. Polymorphisms in the human angiotensin II receptor genes have been linked to cardiovascular and nephrological disorders. In this study we evaluated 35 patients with either primary obstructive megaureter or posterior urethral valves. Each was genotyped for the A1166 AT1 polymorphism and the recently described A-1332G AT2 transition. Objective: Evaluate the use of lingual mucosal graft (LMG) in anterior urethral strictures.Methods: From January 2001 to December 2006, 29 men (mean age, 48.5 yr) with anterior urethral strictures underwent graft urethroplasty with LMG. The mean length of stricture was 3.6 cm. Patients with bulbar, penile, or bulbopenile strictures received one-stage dorsal free graft urethroplasties. In patients with failed hypospadias repair we performed a two-stage urethroplasty. Criteria for successful reconstruction were spontaneous voiding with no postvoid residual urine and no postoperative instrumentation of any kind. Abstract In 14 male dogs, the functional urethra was replaced by a bladder flap tube in a 2-stage procedure. In the 1st stage of the operation a bipedicle strip from the anterior bladder wall was tubularized around a 10F or 12F catheter. After 10 to 12 weeks the 2nd stage of the procedure was performed: the bladder neck was transected and sutured, and the cranial pedicle of the bipedicle tube was transected and pulled through the bluntly dissected pelvic floor muscles. Purpose Buccal mucosa onlay grafts are widely used in complex Hypospadias surgery. In a retrospective study we evaluated the long-term results in patients with a minimum follow-up of 5 years. Material and Methods All patients in who underwent buccal mucosa grafting for Hypospadias repair in our institution received a questionnaire and were invited for a follow-up visit. The questionnaire included two visual analogue scales (VAS) concerning the functional and cosmetic result (0 = very dissatisfied; 50 = satisfied; 100 = very satisfied). Effects of Different Glucose Concentrations on Spinal Anaesthesia with Bupivacaine and Tetracaine

The effects of 5% and 8% glucose in 0.5% tetracaine or bupivacaine on the anaesthetic spread were investigated in 80 urological patients requiring spinal anaesthesia for trans-urethral resection of the prostate. The local anaesthetic solutions were randomly administered, the patients being divided into four groups of 20, and the anaesthetic profile was then evaluated in a double-blind fashion by an independent observer. Maximum cephalad spread of analgesia was significantly greater with tetracaine in 8% glucose compared to the other three groups (tetracaine/5% glucose, bupivacaine/5 or 8% glucose) (P less than 0.05).
