Autonomic dysfunction in patients with achalasia.
It has been previously shown that patients with achalasia may have motor abnormalities of the stomach, small bowel and biliary system. This study investigates whether a disturbance of extraintestinal autonomic function occurs. Autonomic function studies were performed in 15 patients with achalasia and 15 age- and sex-matched healthy controls. Pupillo-grams were obtained during darkness, light exposure and after pilocarpine administration. Cardiovascular function studies included determinations of heart rate variation during deep breathing and orthostasis. Treatment of advanced pancreatic cancer with 5-fluorouracil, folinic acid and interferon alpha-2A: results of a phase II trial.
Interferon alpha-2a (IFN-alpha) and folinic acid (FA) have been shown to modulate the cytotoxic effects of 5-fluorouracil (5-FU) in the treatment of cancer. A phase II study was initiated to evaluate the effect of a combination of 5-FU/FA/IFN-alpha in patients with advanced pancreatic cancer. Sixty previously untreated patients with advanced adenocarcinoma of the pancreas were treated with 500 mg m-2 FU via an intravenous bolus 1 h after the initiation of a 2 h infusion of 500 mg m-2 FA. Before starting the FA infusion, 6 million units (MU) of IFN-alpha was administered subcutaneously. The treatment was repeated once a week. Urinary glycosaminoglycans in Graves' ophthalmopathy.
An increased accumulation of glycosaminoglycans (GAG) in retrobulbar tissues has been reported in patients with thyroid eye disease. We examined the quantitative urinary GAG excretion in 101 patients with Graves' ophthalmopathy of different classes, 36 patients with Graves' hyperthyroidism without ophthalmopathy, 14 patients with toxic nodular goitre and 103 control subjects. Glycosaminoglycans were isolated from 24-h urine collections by precipitation with cetylpyridinium chloride and ethanol followed by photometrical quantification of hexuronic acids after reaction with carbazole. Morbidität von Begleiteingriffen bei der Halsexploration wegen primärem Hyperparathyreoidismus Ergebnisse einer prospektiven Untersuchung
In einer prospektiven Beobachtungsstudie nach operativer Therapie eines primaren Hyperparathyreoidismus wurden die Incidenz durchgefuhrter Begleiteingriffe und deren Einflus auf die Morbiditat der Halsrevision analysiert. Vom 1. 8. 1987 bis 15. 10. 1995 wurden 231 Patienten operiert. Bei 16 Patienten erfolgten simultan 18 (7,8 %) abdominal-, thoraxchirurgische Operationen sowie Eingriffe bei Weichgewebstumoren und bei 133 Patienten (57,6 %) Schilddrusenresektionen unterschiedlichen Ausmases, bei 4,8 % bestanden Schilddrusencarcinome. Short-and long-term outcome after PTCA in patients with stable and unstable angina
Acute results and follow-up data over a period of 36 months after attempted PTCA in 406 patients with stable angina and 202 patients with unstable angina are reported. The rate of acute complications (death, myocardial infarction and bypass grafting (CABG) amounted to 1.5% in stable and 6.4% in unstable patients (P less than 0.005). Within the first week after PTCA a significantly lower percentage (1.7% vs 10.4%) of cardiac events (death, myocardial infarction, CABG and repeat PTCA) was observed in the stable group (P less than 0.001).
