Aceclofenac versus piroxicam in the management of osteoarthritis of the knee: A double-blind controlled study

Abstract The objective of this study was to compare the efficacy and tolerability of aceclofenac tablets 100 mg twice daily with piroxicam tablets 20 mg once daily in the treatment of osteoarthritis of the knee or gonarthritis. A randomized, double-blind, controlled study of 3 months' duration was conducted in patients with gonarthritis diagnosed according to the World Health Organization criteria. A total of 212 patients were recruited; 205 patients fully complied with the inclusion-exclusion criteria. Of these, 103 received aceclofenac and 102 received piroxicam. Fourteen patients in the aceclofenac group and 12 patients in the piroxicam group who failed to complete the 3-month treatment …

Bisphosphonate-associated femoral fracture: implication for management

Studies carried out on individuals being treated long term with bisphosphonates have provoked considerable interest and perplexity about the effect that these drugs have on bone turnover in the long run. In fact the experiences reported by numerous researchers tend to highlight how treatment with high doses of bisphosphonates over many years, of individuals with osteoporosis complicated by or secondary to neoplastic pathologies, causes a suppression of bone turnover that over time predisposes the bone to the accumulation of micro damage that can then result in complicated fractures, as in the case described here.

A case of bilateral luxatio erecta

AbstractLuxatio erecta is an uncommon form of glenohumeral dislocation. Cases of bilateral inferior shoulder dislocation (luxatio erecta) are very rare, and only ten cases have been described in literature. We describe a case of a woman with bilateral luxatio erecta of the shoulders treated with close reduction, immobilization and rehabilitation. The patient had fallen downstairs while clinging to both lateral banisters.

Multilevel functional surgery in adults with acquired spasticity of upper extremity

The authors present their expertise in the treatment of acquired spasticity of the upper extremity in adults, illustrating the basics of modern functional surgery, which benefits from the achievements in bioengineering and information technology and new surgical techniques. Moreover close cooperation between the physical therapist and surgeon is essential in planning a new treatment scheme which, when possible, may successfully strengthen and optimize the function control of the remaining extremity, thus overriding the old concept of merely correcting the deformity. Results depend on proper scrutiny of the preoperative clinical and instrumental details on successful surgical accomplishment …

Aceclofenac versus naproxen in the treatment of ankylosing spondylitis: A double-blind, controlled study

Abstract The efficacy and tolerability of aceclofenac and naproxen sodium in the treatment of ankylosing spondylitis (AS) were evaluated and compared in this double-blind, multicenter, controlled study. Of the 130 patients who entered the study, 126 patients met the inclusion criteria. Efficacy was evaluated at baseline, 15 days, and 1, 2, and 3 months using a visual analog scale for spontaneous pain, a zero to three-point scale for pain on movement and pain at rest, and measurements of chest expansion, hand-to-floor distance, Schober's test, and normal daily activities. No significant between-group differences were seen for any of the variables at baseline, except hand-to-floor distance. B…

Posterior subtalar dislocation.

A subtalar dislocation of the foot is an uncommon injury, and cases of posterior subtalar dislocation are even rarer. This injury is characterized by a simultaneous dislocation of talocalcaneal and talonavicular joints while tibiotalar and calcaneocuboid articulations remain intact. Subtalar dislocation is commonly accompanied by fractures of the malleoli, talus, or fifth metatarsal and by a rotational component of the subtalar joint. Subtalar dislocation can occur in any direction and produces significant deformity. This article presents a case of pure posterior subtalar dislocation in an 80-year-old man who presented with pain and deformity in his left ankle following a motorcycle acciden…

Simultaneous chronic rupture of quadriceps tendon and contra-lateral patellar tendon in a patient affected by tertiary hyperparatiroidism

Abstract Spontaneous ruptures of the extensor mechanism of the knee are very rare. They tend to increase considerably in patients with metabolic diseases such as chronic renal failure, hyperparathyroidism, diabetes, gout, and systemic lupus erythematosus. The reported case regards a 48-year-old man with chronic, spontaneous and simultaneous quadriceps, and contra-lateral patellar tendon rupture. The patient suffered from chronic renal failure and for the past year from tertiary hyperparathyroidism. Ruptured tendons were repaired and both knee were evaluated monthly for the next 12 months. Good functional recovery was achieved on both knees without relapse. This case emphasizes the importanc…

Aceclofenac cream versus piroxicam cream in the treatment of patients with minor traumas and phlogistic affections of soft tissues: a double-blind study

Abstract The efficacy and safety of aceclofenac 1.5% cream and piroxicam 1% cream in 220 patients with soft-tissue injuries were evaluated in a double-blind, randomized, multicenter, comparative, parallel-group trial. Patients were randomly assigned to each treatment group to receive therapy three times a day for 7 days. The primary outcome measure of pain was evaluated using a visual analog scale (VAS) at baseline, day 4, and day 7 of the study. Safety was assessed by recording any adverse events that occurred and by monitoring any hemato-biochemical alterations during the study. After days 4 and 7 of treatment, a significant improvement ( P P

Intramuscular tramadol versus ketorolac in patients with orthopedic and traumatologic postoperative pain: a comparative multicenter trial

This 3-day, multicenter, open-label, randomized, controlled trial was undertaken to assess the postoperative analgesic effect and therapeutic safety of intramuscular tramadol compared with intramuscular ketorolac in 48 patients undergoing orthopedic surgery and having postoperative pain assessed as 75 mm or more on a 100-mm visual analogue scale. Tramadol (100 mg/2 mL ampules) was administered as needed to a maximum of 400 mg/d, and ketorolac (30 mg/1 mL ampules) was given as needed up to 90 mg/d. Nine patients who received tramadol and six who received ketorolac needed only one dose of medication to achieve satisfactory analgesia. Tramadol had a more pronounced analgesic effect than did ke…
