Pasireotide treatment reduces cardiometabolic risk in Cushingâs disease patients: an Italian, multicenter study
Purpose: Patients with Cushing’s disease (CD) experience metabolic alterations leading to increased cardiovascular mortality. Recently, the visceral adiposity index (VAI) has been proposed as a marker of visceral adipose tissue dysfunction (ATD) and of the related cardiometabolic risk. We aimed to evaluate the impact of 12-month pasireotide treatment on cardiometabolic risk in CD patients. Methods: This is a multicentre, prospective, and observational study. Sixteen CD patients, referred to the Endocrine Units of the University Hospitals of Messina, Napoli, Padova, and Palermo (Italy), successfully treated with pasireotide for 12 month have been enrolled. In all patients, we assessed anthro…
Epidemiology of pancreatic neuroendocrine neoplasms. a gender perspective
Purpose: Pancreatic neuroendocrine neoplasms (PNENs) are a group of clinically rare and heterogeneous tumors of the pancreas. Currently there are no studies investigating the gender difference in PNEN susceptibility. Thus, the purpose of this study was aimed at examining how gender shapes risk factors, clinicopathological features, and comorbidities in PNENs. Methods: The study design consisted of an Italian multicenter, retrospective study. The study included all consecutive patients with PNENs followed at the participating centers. Two hundred and twenty-nine patients (105 males,124 females, age 54 ± 0.98 years) with PNENs were enrolled at the participating centers. The clinicopathologica…
The Metabolic Profile in Active Acromegaly is Gender-Specific.
CONTEXT: The sexual dimorphism of the somatotroph axis has been documented, but whether the acromegaly-related metabolic alterations are gender-dependent has never been investigated. OBJECTIVE: The aim of the study was to evaluate the impact of gender on the metabolic parameters in acromegaly. DESIGN: We conducted a retrospective, comparative, multicenter study. PATIENTS: The 307 newly diagnosed acromegalic patients included in the study were grouped by gender: 157 men (aged 48.01 ± 14.28 yr), and 150 women (aged 48.67 ± 14.95 yr; of which 77 were premenopausal and 73 postmenopausal). OUTCOME MEASUREMENTS: We measured each component of the metabolic syndrome (MS), hemoglobin A1c, the areas …
The treatment of hyperinsulinemic hypoglycaemia in adults: an update
Treatment of hyperinsulinemic hypoglycaemia (HH) is challenging due to the rarity of this condition and the difficulty of differential diagnosis. The aim of this article is to give an overview of the recent literature on the management of adult HH. A search for reviews, original articles, original case reports between 1995 and 2016 in PubMed using the following keywords: hyperinsulinemic hypoglycaemia, insulinoma, nesidioblastosis, gastric bypass, autoimmune hypoglycaemia, hyperinsulinism, treatment was performed. One hundred and forty articles were selected and analysed focusing on the most recent treatments of HH. New approaches to treatment of HH are available including mini-invasive sur…
Glycometabolic control in Acromegalic patients with Diabetes: a study of the effects of different treatments for GH excess and for hyperglycaemia.
Background. Diabetes mellitus (DM) is frequently observed in patients with acromegaly. Current therapies for acromegaly may impact glucose regulation, influencing insulin sensitivity and secretion. The question whether these therapies modify control and progression of diabetes once present is still open. Aim. Aim of our study is to analyse glucose control in acromegalic patients with diabetes, evaluating the relation with treatments for GH excess and for diabetes. Methods. Seventy patients with acromegaly and diabetes were studied. Duration and treatments of acromegaly and DM were recorded, together with clinical and metabolic parameters. Results. Most patients (92.8%) were treated with som…
Effect of the switch from conventional to “dual release hydrocortisone” in adult patients with primary and secondary adrenal insufficiency: a six-months multicenter study
Evidence for altered adipocyte function in polycystic ovary syndrome.
Background: Adipocytokines are produced by adipose tissue and have been thought to be related to insulin resistance and other health consequences. We measured leptin, adiponectin, and resistin simultaneously in women with polycystic ovary syndrome (PCOS) and age- and weight-matched controls. Our hypothesis was that these simultaneous measurements would help determine whether adipocytokine secretion is abnormal in PCOS independent of body mass and whether these levels are related to insulin resistance as well as other hormonal changes. Methods: Fifty-two women with PCOS and 45 normal ovulatory women who were age- and weight-matched were studied. Blood was obtained for adipocytokines (leptin,…
Assessment of the awareness and management of sleep apnea syndrome in acromegaly. The COM.E.TA (Comorbidities Evaluation and Treatment in Acromegaly) Italian Study Group
In 2007 the Italian COM.E.T.A. (COMorbidities Evaluation and Treatment in Acromegaly) study group started to assess the application in a clinical setting of the Versailles criteria for management of acromegaly complications by a first questionnaire focusing on cardiovascular co-morbidities. A further questionnaire on sleep apnea syndrome (SAS) was delivered by the COM.E.T.A. study group to 107 endocrine centers in Italy. The results of our survey suggest that SAS is a well-known comorbidity even if its estimated prevalence is lower than in the literature. Polysomnography is the preferred tool for diagnosis. Control of SAS is considered relevant both for quality of life and co-morbidities. C…
EFFECTS OF PASIREOTIDE TREATMENT ON CARDIOMETABOLIC RISK IN PATIENTS WITH CUSHING’S DISEASE : AN ITALIAN, MULTICENTER STUDY
HIGH VISCERAL ADIPOSITY INDEX (VAI) SCORE BEFORE AND AFTER TREATMENT IN YOUNGH HIV-POSITIVE PATIENTS
The new therapies in HIV-positive patients are extremely effective in terms of immunological response. In western countries the mortality rate was reduced by 100% from 1984 to the current 8.8%. HIV people are now patients with a chronic disease characterized by also metabolic disorders. Long term virus alterations and treatment sideeffects induced lipid disorders, in particular hypercholesterolemia and hypertriglyceridemia. To assess cardiovascular risk (CVR) in HIV patients. From April 2012 to January 2013 we evaluated 20 HIV-positive naive patients, no HBV-HCV coinfected (19 males, age range 21-42 years, mean 31.8 + 4.8, BMI 23.5 + 3.8). Before and after inhibitor proteases (IP) treatment…
Endothelial dysfunction in PCOS. role of obesity and adipose hormones
PURPOSE: Polycystic ovary syndrome (PCOS) is an extremely prevalent disorder in which elevated blood markers of cardiovascular risk and altered endothelial function have been found. This study was designed to determine if abnormal carotid intima-media thickness (IMT) and brachial flow-mediated dilation (FMD) in young women with PCOS may be explained by insulin resistance and elevated adipocytokines. METHODS: A prospective study in 50 young women with PCOS (age: 25.2 +/- 1 years; body mass index [BMI]: 28.7 +/- 0.8) and 50 matched ovulatory controls (age: 25.1 +/- 0.7 years; BMI: 28.5 +/- 0.5) was performed. Carotid IMT, brachial FMD, and blood for fasting glucose, insulin, leptin, adiponect…
