Theories and methods to assess the social cost of occupational risk factors in France : application to the case of occupational cancers

The main objective of this thesis is to evaluate the social cost of respiratory cancer attributable to occupational risk factors in France. A summary of the available epidemiological data in the literature is performed. By using the Medline database, a review of the literature restricted to meta-analysis highlights the relative risk data available. The method of Attributable Risks (AR) is mobilized to estimate the numbers of lung, sinonasal and mesothelioma cancer cases caused by asbestos, exhaust fumes from diesel engines, painters, crystalline silica, wood dust and leather dust. Classical power computation can be used to derive the minimal number of subjects of a sensory panel in order to control both type I (α risk) and type II (β risk) errors. Two different techniques aimed to assess the appropriateness of hospital utilisation were used to study the same sample of clinical records of admissions provided during 2001 by the emergency room of a pediatric sicilian hospital. The first technique estimated the '"high risk of inappropriateness" DRGs prevalence, according to the Decreto del Presidente del Consiglio dei Ministri of 29 Novembre 2001, and the second was the pediatric version of the Appropriateness Evaluation Protocol (AEP), based on clinical characteristics of patients and complexity of assistance provided.
