Incarcerated hernia in a trocar site following laparoscopic gastric bypass

This paper reports a case of small bowel occlusion due to the herniation of an ileal ansa at the site of the insertion of a 12-mm trocar. A 28-year-old obese female patient underwent laparoscopic surgery for gastric bypass, the skin incisions of the trocar insertion sites were closed with absorbable sutures. Small bowel occlusion occurred on post-operative day 6 and the patient again underwent laparoscopic surgery. Laparoscopic exploration confirmed the suspected presence of the incarcerated hernia of an ileal ansa at the site of the trocar insertion which was freed without any need for bowel resection. Introduction The terminal ileum is the most involved tract in Crohn's disease. The obstruction in this location is the most frequent complication. Acute or chronic presentations can occur. Surgery finds a role in the management of chronic strictures and in acute clinical presentations with complications not improving with conservative therapy. Abstract Background Steroids, the mainstay of Crohn's disease treatment, have been associated with systemic side effects. Aim To evaluate the efficacy and tolerability of beclomethasone dipropionate for maintaining remission induced by a short course of systemic steroids in patients with Crohn's ileitis with or without right colonic involvement. Usefulness of ileostomy defunctioning stoma after anterior resection of rectum on prevention of anastomotic leakage A retrospective analysis

Anastomotic leakage is one of the major complications occurring after anterior resection of rectum. A defunctioning stoma is usually created routinely or on surgeons' discretion. The aim of this study was to investigate the usefulness of temporary ileostomy to prevent anastomotic leakage comparing the postoperative course of patients with and without defunctioning loop ileostomy.Patients that underwent anterior resection of rectum were recruited. 140 patients were enrolled and divided in two groups: patients without and with defunctioning loop ileostomy. Antispasmodic effect of 4'-methylepigallocatechin on guinea pig ileum.

AbstractThe antispasmodic effect of 4′-methylepigallocatechin (MEC), which was isolated from Maytenus rigida Mart (Celestraceae), was investigated in vitro in guinea pig intestinal segments. In the isolated ileum, MEC (1 nM–100μM) did not modify the ileal spontaneous tonus or the electrically elicited contractions. MEC (8μM) significantly (p<0.01) reduced the submaximal contractions induced by histamine (2μM), carbachol (100μM) and BaCl2 (0.03M). An additive relaxing action (p<0.001) was observed by co-incubation of verapamil (10 nM) and MEC (8μM). 1200 years of decadal-scale variability of Mediterranean vegetation and climate at Pantelleria Island, Italy.

A new sedimentary sequence from Lago di Venere on Pantelleria Island, located in the Strait of Sicily between Tunisia and Sicily was recovered. The lake is located in the coastal infra-Mediterranean vegetation belt at 2 m a.s.l. Pollen, charcoal and sedimentological analyses are used to explore linkages among vegetation, fire and climate at a decadal scale over the past 1200 years. A dry period from ad 800 to 1000 that corresponds to the 'Medieval Warm Period' (WMP) is inferred from sedimentological analysis. Die Substanzen zeigten am isolierten Rattendarm eine kompetitiv-nichtkompetitive anticholinerge Wirkung, am isolierten Meerschweinchendarm eine kompetitiv-nichtkompetitive Antihistaminwirkung. An isolierten Meerschweinchenvorhofen trat nach Gabe der tertiaren 2-Skatyl-alkylpyridine eine durch Atropin aufzuhebende negativ inotrope Wirkung auf, die quartaren Substanzen waren dagegen positiv inotrop wirksam.
