Slightly more complex than the previous one, a quick way to induce hyperemia is to inject a contrast bolus. Although the degree of hyperemia reached this way is not as large as with vasodilators, this maneuver can still allow unmasking a gradient.

There are several ways to reopen an acutely occluded coronary artery. Thrombolysis can be achieved with various methods (1, 5, 7, 10). After thrombolysis we find that the residual thrombus can sometimes be seen, but in most cases, a more or less high grade coronary stenosis remains (2). It is also a question of whether it is always optimal to reopen the vessel, because if the vessel remains occluded, another occlusion cannot occur. With this situation one faces the possibility that the patient will suffer another infarction, and that the vessel will occlude again (4).

Aims In low-flow, low-gradient aortic stenosis (LF/LG AS), the assessment of contractile reserve (CR) by dobutamine stress echocardiography (DSE) is recommended for risk stratification and treatment strategy. However, DSE may show limitations in cases of left ventricular dyssynchrony (LVD). The impact of LVD in LF/LG AS, and the feasibility of CRT in this setting have never been evaluated. Subglottic stenosis imitating the carina – a case report of airway mimicry

Summary We present a case of awake tracheal intubation with flexible bronchoscopy which resulted in incorrect tracheal tube placement. The presence of a stenotic subglottic lesion with an appearance similar to the carina led to the tube being positioned with only the tip within the trachea whilst the cuff was located between the vocal cords. A capnography trace was identified before induction of anaesthesia; however, visual confirmation of the carina was undertaken in a rushed manner due to the patient becoming agitated. Significance of ultrasound for the diagnosis of hypertrophic pyloric stenosis

The sonographic features of hypertrophic pyloric stenosis (HPS) were evaluated in a 5-year prospective study (1981–1985) at the University Clinic of Pediatric Surgery in Mainz. In 37 cases real-time ultrasound was performed preoperatively to measure muscle thickness, diameter, and length of the pylorus; the recorded values were compared with those of a control group. Muscular wall thickness proved to be the most reliable parameter, while pyloric canal length was the most difficult measurement to obtain.
