Parathyroid hormone serum concentration kinetic profile in critically ill patients undergoing continuous renal replacement therapies: a prospective o…

2021

Introduction: Elevated serum parathormone (PTH) levels have been observed in acute kidney injury and are related to calcium-phosphate metabolism disturbance, decreased renal production of 1,25 dihydroxyvitamin D3, impaired renal PTH excretion, and other renal-independent factors. There are no data regarding PTH concentration kinetics in critically ill patients undergoing continuous renal replacement therapies (CRRT) in an intensive care setting. The primary objective of this study was to investigate trends in PTH serum levels in critically ill patients with multiorgan failure undergoing CRRT, by performing periodic PTH measurements in the acute phase of critical illness. Material and method…

Management of Blood Pressure and Heart Rate in Patients with Acute Stroke

2017

BACKGROUND: Stroke represent one of the most devastating of all neurological diseases, affecting about 15 million people per year and is an important cause of morbidity and mortality worldwide and currently the leading cause of adult disability in developed countries. Blood pressure and heart rate may undergo several modifications in patients with both ischemic and hemorrhagic stroke in fact raised blood pressure levels may lead to cerebral edema, hematoma expansion or hemorrhagic transformation and in contrast low blood pressure can lead to increased cerebral infarction or perihematomal ischemia. In addition, ECG abnormalities and cardiac arrhythmias, especially atrial fibrillation, are re…

Is acute heart failure a distinctive disorder? An analysis from BIOSTAT-CHF

2020

Aims: \ud This retrospective analysis sought to identify markers that might distinguish between acute heart failure (HF) and worsening HF in chronic outpatients.\ud \ud Methods and Results: \ud The BIOSTAT‐CHF index cohort included 2516 patients with new or worsening HF symptoms: 1694 enrolled as inpatients (acute HF) and 822 as outpatients (worsening HF in chronic outpatients). A validation cohort included 935 inpatients and 803 outpatients. Multivariable models were developed in the index cohort using clinical characteristics, routine laboratory values, and proteomics data to examine which factors predict adverse outcomes in both conditions and to determine which factors differ between ac…

AGE DIFFERENCES IN THE ACUTE STRESS EFFECTS ON DECLARATIVE MEMORY PERFORMANCE

2021

"In the last decades, there has been a growing interest in knowing the effects of acute stress on memory performance, particularly declarative memory. Research on this topic suggests that age is a crucial individual factor to consider in the stress-memory link. However, most of the evidence has been obtained from studies conducted in young people and, surprisingly, studies in older people are scarce. Thus, our aim was to investigate the age differences in the acute stress effects on declarative memory performance. To do this, we directly compared the effects of a psychosocial acute stressor (i.e. Trier Social Stress Test) on learning, consolidation and memory retrieval performance in two ag…

Hereditary Angioedema: Increased Number of Attacks after Frequent Treatments with C1 Inhibitor Concentrate

2009

OBJECTIVE: C1 inhibitor concentrate is regarded as effective and safe in treating acute attacks of hereditary angioedema caused by C1 inhibitor deficiency. This study investigated the course of disease in 3 women treated frequently with C1 inhibitor concentrate. METHODS: Three women are described who received C1 inhibitor concentrate for the treatment of acute attacks of hereditary angioedema and experienced an increase in the frequency of attacks during that treatment period. In a control group of 24 patients aged more than 60 years with hereditary angioedema, the natural course of disease was determined. RESULTS: The 3 women (ages 50, 69, and 72 years) had received C1 inhibitor concentrat…

Impact of Acute Hemoglobin Falls in Heart Failure Patients: A Population Study

2020

Aims: This study assessed the impact of acute hemoglobin (Hb) falls in heart failure (HF) patients. Methods: HF patients with repeated Hb values over time were included. Falls in Hb greater than 30% were considered to represent an acute episode of anemia and the risk of hospitalization and all-cause mortality after the first episode was assessed. Results: In total, 45,437 HF patients (54.9% female, mean age 74.3 years) during a follow-up average of 2.9 years were analyzed. A total of 2892 (6.4%) patients had one episode of Hb falls, 139 (0.3%) had more than one episode, and 342 (0.8%) had concomitant acute kidney injury (AKI). Acute heart failure occurred in 4673 (10.3%) patients, represent…

Fungal Necrotizing Scleritis After Intravitreal Injection Therapy

2020

PURPOSE To report a case of infectious necrotizing scleritis secondary to Aspergillus terreus after intravitreal injection therapy. METHODS This is a case report with literature review. RESULTS A 98-year-old woman receiving intravitreal aflibercept injections for neovascular age-related macular degeneration in the left eye presented with severe pain, redness, and purulent discharge at the injection site. She was initially treated with topical fortified antibiotics, and clinical improvement was achieved, although microbial cultures showed negative results. Two months later, she presented with severe ocular pain and was diagnosed with anterior necrotizing scleritis. Scleral scrapings were col…

High-Sensitivity Procalcitonin (hs-PCT): A Marker for Identification of Arteriosclerosis and Myocardial Infarction?

2008

Background: Procalcitonin (PCT) is a protein hormone induced by bacterial inflammation that serves as a marker of sepsis. C-reactive protein (CRP) is also known to be a marker of inflammation and, in low concentrations, a prognostic marker for latent or early arteriosclerosis. Arteriosclerosis is considered as an inflammatory process that begins in the wall of an artery. In addition to CRP, the concentration of other markers of inflammation increases in the presence of arteriosclerosis. Methods: To determine if PCT is among these markers, the serum concentration of PCT was quantified in blood samples from healthy individuals (n = 410) and at admission in 774 patients, 92 with peripheral art…

Les anti-agrégants plaquettaires au cours du syndrome coronarien aigu. Données des grandes études

2010

Cardiovascular disease is the leading cause of early death and morbidity in developed countries and is becoming a growing problem in many developing countries. Currently, anti-coagulants play a major role in the management of the acute phase in association or not with reperfusion strategies. The combination of clopidogrel and aspirin, in accordance with the results of large randomized trials, is the treatment of reference. However two new drugs, a thienopyridine (prasugrel), and a cyclo-pentyl-triazolo-pyridine (ticagrelor) have shown their interest in major studies in comparison with clopidogrel. As a result, several effective P2Y12 receptor antagonist anti-platelet agents are now availabl…
