Outpatient minor oral surgery in patients with hemophilia: A case series of 23 patients

2019

Hemophilia is an inherited coagulation disorder characterized by deficiency of the coagulation factor VIII or IX. When the management of hemostasis is inadequate, these patients are at high risk of experiencing intra and postoperative bleedings after tooth extractions. Coagulation factor replacement therapy allows performing most surgical procedures safely, although the factor levels and length of treatment have not been clearly determined. In this study, we present our experience in a retrospective series of 112 ambulatory tooth extractions under local anesthesia in 23 patients with hemophilia using a coagulation factor replacement therapy in combination with tranexamic acid. The results o…

The use of a questionnaire for improvement of postoperative analgesia after transurethral resection of the prostate

2003

Abstract Background : Postoperative analgesic studies with methods like i.v. patient controlled analgesia (i.v. PCA) or epidural analgesia have reported effective pain relief after major surgery, also after urologic cancer surgery. In contrast, systematic results after minor urologic surgery are widely unknown, although the pain intensities may be high for a short time. The aim of the study was to evaluate the usefulness of a pain questionnaire as a measurement tool of postoperative analgesic quality after transurethral resection of the prostate (TUR-P). Methods : A questionnaire of the American Pain Society (APS) for quality assurance of postoperative pain therapy was adapted to estimate t…

Gastrointestinal disorders and dabigatran.

2012

Anticoagulants play an important role in the prevention and treatment of a variety of acute and chronic thromboembolic disorders such as primary prevention and treatment of venous thromboembolism or prevention of stroke and systemic embolism in atrial fibrillation just to name of few. Within recent years, a promising new oral anticoagulant, the direct thrombin inhibitor dabigatran etexilate (dabigatran) successfully underwent clinical development and has emerged as an alternative to vitamin K antagonists according to a variety of recently revised and updated international guidelines referring to the indication of stroke prevention in atrial fibrillation. Considering the intensive clinical u…

Synchronization index for quantifying nonlinear causal coupling between RR interval and systolic arterial pressure after myocardial infarction

2002

The analysis of nonlinear couplings among cardiovascular variability series can improve the knowledge of the cardioregulatory mechanism and help to understand how it can be affected by pathologies. In this study, the influences of acute myocardial infarction (AMI) on the causal relationships between the heart period and the arterial pressure were investigated by a nonlinear dynamic approach based on the corrected cross-conditional entropy. Whereas the global synchronization index did not differentiate the post-AMI patients from the young and old control groups, the causal indexes evidenced the impairment of the baroreflex control and showed an increase of the mechanical driving of the RR in…

A prospective randomized clinical trial compared the effect of various types of local anesthetics cartridges on hypertensive patients during dental e…

2015

Objectives: To evaluate hemodynamic changes of blood pressure and heart rate on hypertensive patients undergoing tooth extraction using various types of local anesthesia (LA). Study Design: A prospective randomized clinical trial was conducted on 45 hypertensive patients who were divided equally into 3 parallel groups according to LA received. Group 1: Lidociane 2% with epinephrine 1: 80,000. Group 2: Prilocaine 3% with Felypressin 0.03 IU/ml. Group 3: Mepivacaine 3% plain. Inclusion criteria: hypertensive patients, under medical management with blood pressure ≤ 159/99. Exclusion criteria: Blood pressure ≥160 /100 and patients receiving β blockers. Negative aspiration was mandatory before t…

Electrical Stimulation of Skeletal Muscles An Alternative to Aerobic Exercise Training in Patients With Chronic Heart Failure?

2006

The aim of this study was to investigate whether electrical stimulation of skeletal muscles could represent a rehabilitation alternative for patients with chronic heart failure (CHF). Thirty patients with CHF and NYHA class II-III were randomly assigned to a rehabilitation program using either electrical stimulation of skeletal muscles or bicycle training. Patients in the first group (n = 15) had 8 weeks of home-based low-frequency electrical stimulation (LFES) applied simultaneously to the quadriceps and calf muscles of both legs (1 h/day for 7 days/week); patients in the second group (n = 15) underwent 8 weeks of 40 minute aerobic exercise (3 times a week). After the 8-week period signifi…

<title>Near-infrared spectroscopy (NIRS) as a diagnostic tool in patients with suspected stroke or traumatic brain injury</title>

2001

Near-Infrared Spectroscopy (NIRS) as a diagnostic tool in patients with suspected stroke or brain injury S. Goldberg, C. Lott, M. Ostermeyer, H.-J. Hennes Absorption of Near-Infrared (NIR) light in the brain is mainly caused by hemoglobin. Superficial intracranial hematoma with a higher concentration of hemoglobin causes a higher absorption in NIRS. The existence of hemorrhage can be demonstrated by the difference of optical density, comparing identical measuring points at both hemispheres of the brain: absorption of NIR light is greater at the side of the hemorrhage, causing less reflection in NIRS. In a prospective, blinded study, 100 patients who were scheduled for CCT-scan for brain inj…

Long-term safety, tolerability and efficacy of flexible-dose fesoterodine in elderly patients with overactive bladder: Open-label extension of the SO…

2013

Aims To assess the long-term safety, tolerability, and efficacy of flexible-dose fesoterodine in elderly patients with OAB. Methods Patients aged ≥65 years who completed a 12-week, randomized, double-blind, placebo-controlled trial were eligible for the 12-week, open-label (OL) extension phase. Patients who received double-blind placebo started on fesoterodine 4 mg and could increase to 8 mg after 4 or 8 weeks of OL treatment, while fesoterodine-treated patients continued on their double-blind dose; only one dose escalation or de-escalation was permitted. Discontinuations and adverse events (AEs) were monitored, and patients completed 3-day bladder diaries and patient-reported outcomes at t…
