Successful Foley catheter tamponade of an epigastric vessel trocar injury in a toddler – A case report

Highlights • Epigastric vessel injury is a well recognized complication during laparoscopic trocar placement. • Precautionary measures to prevent trocar induced injuries should be implemented to lower the incidence of this complication. • If this complication is encountered, tamponade by Foley catheter placement is an effective measure to provide hemostasis. • Hemorrhage control using a Foley catheter is feasible even in small children and toddlers.

Management of Bleeding Complications in Virtual Reality Laparoscopy

The aim of this study was to compare the impact of induced bleeding complication training with regular training on a virtual reality laparoscopic (VRL) simulator. Although bleeding complications occur rarely during laparoscopic surgery, they usually arise without warning and may have severe consequences for the patient because complication management training is not currently widespread. Recombinant activated factor VII administration in a patient with congenital lack of factor VII undergoing laparoscopic hysterectomy: A case report

Highlights • Patients with lack of factor VIIa, suffering from abnormal uterine bleedings can be treated with laparoscopic hysterectomy. • Technique using bipolar coagulation and non-absorbable clips is safe and not time consuming. • Pre-operative loading with recombinant factor VIIa must be followed by prolonged supplementation in post-operative period. • We present suggested dosage and length of factor VIIa supplementation based on literature review and own experience.

Hemostatic balance in patients with liver cirrhosis: Report of a consensus conference.

Abstract Patients with cirrhosis present with hemostatic alterations secondary to reduced availability of pro-coagulant and anti-coagulant factors. The net effect of these changes is a rebalanced hemostatic system. The Italian Association of the Study of the Liver (AISF) and the Italian Society of Internal Medicine (SIMI) promoted a consensus conference on the hemostatic balance in patients with cirrhosis. Emergency sclerotherapy versus vasoactive drugs for variceal bleeding in cirrhosis

Abstract Background & aims: Emergency sclerotherapy is used as a first-line therapy for variceal bleeding in cirrhosis, although pharmacologic treatment stops bleeding in most patients. We performed a meta-analysis comparing emergency sclerotherapy with pharmacologic treatment. Methods: MEDLINE (1968–2002), EMBASE (1986–2002), and the Cochrane Library (2002;4) were searched to retrieve randomized controlled trials comparing sclerotherapy with vasopressin (± nitroglycerin), terlipressin, somatostatin, or octreotide for variceal bleeding in cirrhosis. Liver disease is a frequent cause of haemostatic abnormalities, which may lead to overt or occult bleeding. Clinical manifestations of hepatic coagulopathy include upper and lower gastrointestinal haemorrhage, easy bruising and bleeding from gums, nose or the female genital tract. The most significant bleeding problem among patients with chronic liver disease is blood loss due to portal hypertension.
