Max–min dispersion with capacity and cost for a practical location problem

2022

Diversity and dispersion problems deal with selecting a subset of elements from a given set in such a way that their diversity is maximized. This study considers a practical location problem recently proposed in the context of max–min dispersion models. It is called the generalized dispersion problem, and it models realistic applications by introducing capacity and cost constraints. We propose two effective linear formulations for this problem, and develop a hybrid metaheuristic algorithm based on the variable neighborhood search methodology, to solve real instances. Extensive numerical computational experiments are performed to compare our hybrid metaheuristic with the state-of-art heurist…

On one-dimensionality of metric measure spaces

2019

In this paper, we prove that a metric measure space which has at least one open set isometric to an interval, and for which the (possibly non-unique) optimal transport map exists from any absolutely continuous measure to an arbitrary measure, is a one-dimensional manifold (possibly with boundary). As an immediate corollary we obtain that if a metric measure space is a very strict $CD(K,N)$ -space or an essentially non-branching $MCP(K,N)$-space with some open set isometric to an interval, then it is a one-dimensional manifold. We also obtain the same conclusion for a metric measure space which has a point in which the Gromov-Hausdorff tangent is unique and isometric to the real line, and fo…

Oral contraceptives in the prevention of endometrioma recurrence: does the different progestins used make a difference?

2013

""The primary aim of the study was to analyze the endometrioma recurrence rate in patients who underwent laparoscopic excision followed by postoperative long-term regimen of oral contraceptives (OCs).. . MATERIALS AND METHODS: 168 patients who underwent a conservative laparoscopic surgery for endometrioma, during the period between September 2009 and August 2010 in three university hospitals were studied. A long-term OCs therapy was offered to all women following surgery. Patients were randomly divided into three groups according to different progestins used (desogestrel, gestodene, dienogest). Women who refused a postoperative hormonal therapy served as control. Follow-up visits and transv…

Monoklonālās antivielas - mūsdienu ārstēšanas iespējas

2018

Myostatin/activin blocking combined with exercise reconditions skeletal muscle expression profile of mdx mice

2015

Duchenne Muscular Dystrophy is characterized by muscle wasting and decreased aerobic metabolism. Exercise and blocking of myostatin/activin signaling may independently or combined counteract muscle wasting and dystrophies. The effects of myostatin/activin blocking using soluble activin receptor-Fc (sActRIIB-Fc) administration and wheel running were tested alone or in combination for seven weeks in dystrophic mdx mice. Expression microarray analysis revealed decreased aerobic metabolism in the gastrocnemius muscle of mdx mice compared to healthy mice. This was not due to reduced home-cage physical activity, and was further downregulated upon sActRIIB-Fc treatment in enlarged muscles. However…

Adalimumab in active ulcerative colitis: A “real-life” observational study

2013

Abstract Background and aims The effectiveness of adalimumab in the treatment of ulcerative colitis is under debate. Although controlled trials have shown that adalimumab is significantly better than placebo, the absolute clinical benefit is modest. We report data on the effectiveness of adalimumab in a cohort of ulcerative colitis patients treated in 22 Italian centres. Methods All patients with active disease treated with adalimumab were retrospectively reviewed. Co-primary endpoints were clinical remission at weeks 4, 12, 24 and 54. Secondary endpoints were sustained clinical remission, steroid discontinuation, endoscopic remission and need for colectomy. Results Eighty-eight patients we…

Infliximab, azathioprine or combination therapy in the treatment of active Crohn's disease.

2010

The treatment of moderate-to-severe steroid-dependent Crohn's disease has been a challenging issue for the gastroenterologist. Until to 10 years ago, immunosuppressors, mainly azathioprine or 6-mercaptopurine, were the best possible treatments available, and remission rates ranged from 30 to 60% according to the results of open studies and controlled trials. Recently, infliximab has been shown to be effective in steroid-dependent patients and, combined with azathioprine, it has been shown to increase 1-year remission rates. A recent large randomized controlled trial comparing infliximab, azathioprine and infliximab plus azathioprine demonstrated that the combination of infliximab plus azath…

Spinal anaesthesia in a patient with post-spine surgery dural ectasia.

2013

Dural sac ectasia is a very infrequent anatomical abnormality, usually caused by connective tissue diseases, as Marfan syndrome. Very few cases have been described being a consequence of a previous spine surgical procedure. We describe the case of an elderly patient who should be operated on twice due to sub-occlusive colon disease. Surgery was performed under spinal anaesthesia. A dural sac ectasia was suspected after the first procedure and the abdominal X-ray was reviewed. The characteristics of the anatomical alteration and the course of both anaesthetic procedures were described. X-ray and CT images were provided.
