The composition of human breast milk is highly variable, and it can be influenced by genetics, diet, lifestyle, and other environmental factors. This study aimed to investigate the impact of geographical location and mode of delivery on the nuclear magnetic resonance spectroscopy (NMR) metabolic profile of breast milk and its relationship with the milk microbiome. Human milk metabolic and microbiota profiles were determined using NMR and 16S rRNA gene sequencing, respectively, in 79 healthy women from Finland, Spain, South Africa, and China. Summary Background HELLP syndrome is a pregnancy-related liver disease associated with increased maternal and foetal mortality. In rare cases, it can lead to the development of a subcapsular hepatic haematoma as well as its rupture. This rupture is life-threatening if not urgently treated. Method We describe a clinical case of HELLP syndrome involving a ruptured subcapsular liver haematoma and contextualise this with a literature overview. The exemplum, one of the tools of persuasion indicated by ancient rhetoric, is used by Cicero in Caesarian Orations with a careful strategy: the exemplary modeling of Caesar allows to define the boundaries of the correct management of res publica. Therefore, in the Pro Marcello Caesar is already presented as a “visible” exemplum, while in the following Pro Ligario the paradigm of Sulla (i.e. that of a dictator guided by crudelitas) becomes effective in admonishing Caesar from a possible degeneration of power.

Objective To examine fear of childbirth according to parity, gestational age, and obstetric history. Design A questionnaire study. Population and setting 1400 unselected pregnant women in outpatient maternity clinics of a university central hospital. Methods Visual analogue scale (VAS) and Wijma Delivery Expectancy/Experience Questionnaire (W-DEQ) and preferred mode of delivery. Main outcome measures W-DEQ and VAS scores according to parity, gestational age, obstetric history, and preferred mode of delivery.
