Objective. The aim of the study was to compare the two approaches to chronic right ventricular pacing currently adopted in clinical practice: right ventricular apical (RVA) and non-RVA pacing. In a series of in vitro studies, glial cell volume changes during defined and strictly controlled alterations of the extracellular environment in vitro. Results of heart retransplantation: subanalysis of the Spanish Heart Transplant Registry

Heart retransplantation (ReHT) is controversial in the current era. The aim of this study was to describe and analyze the results of ReHT in Spain. Methods We performed a retrospective cohort analysis from the Spanish Heart Transplant Registry from 1984 to 2018. Data were collected on donors, recipients, surgical procedure characteristics, immunosuppression, and survival. The main outcome was posttransplant all-cause mortality or need for ReHT. We studied differences in survival according to indication for ReHT, the time interval between transplants and era of ReHT. AV-Blocke sind beim akuten Koronarsyndroms selten. Nur wenige Patienten benotigen eine Schrittmacherimplantation. Besonders beim Hinterwandinfarkt ist die Prognose gut. Die Haufigkeit anhaltender AV-Blockierungen hat durch die Akut-PTCA abgenommen. Ventrikulare Tachyarrhythmien sind in der Akutphase des Infarktes haufig. Treten sie als anhaltende Kammertachykardien oder Kammerflimmern > 48 Stunden nach Infarkt auf, konnen sie eine Indikation zur sekundarprophylaktischen ICD-Implantation darstellen. Die Prinzipien der radikalchirurgischen Behandlung von Coloncarcinomen umfassen 1. die En-bloc-Resektion von Tumor, regionalem Lymphabflussgebiet und adharenten Organen sowie 2. die Vermeidung einer operationsbedingten Tumorverschleppung. Hierzu dienen verschiedene Verfahren, deren Wert letztlich nicht erwiesen ist, deren Anwendung jedoch das Risiko des Eingriffs nicht erhoht. Voraussetzung fur die Beurteilung des radikalchirurgischen Vorgehens beim Coloncarcinom ist eine standardisierte operative Technik und eine standardisierte pathohistologische Beurteilung.

163. Die manuelle und maschinelle Bronchusnaht ?Ergebnisse einer konsekutiven Untersuchung

216 elektive Lungenresektionen, die wir zwischen dem 1. 1. 1975 und dem 31. 5. 1986 durchfuhrten, wurden bzgl. der Komplikationsrate nach manuellem (n = 85) bzw. maschinellem Bronchusverschluss und Lungenparenchymnaht (n = 131, seit Nov. 1980) analysiert. Intraoperativ versagte das Klammernahtgerat 4mal (3,1%), so dass die partiell undichten Bronchusstumpfe manuell versorgt werden mussten. Von 5 Parenchymfisteln (3,8%) entwickelte sich lediglich eine nach maschineller Bronchusnaht. Durch die Klammemaht konnten wir die Rate der Bronchusstumpfinsuffizienzen auf 2,9%, verglichen mit 7,5% bei manueller Technik, senken. Parallel fiel die Letalitat von 9,4% auf 3,1%.

Die modernen diagnostischen Untersuchungen wie CT und Sonographie kombiniert mit der PTCD fuhren zu einem Wandel des diagnostisch therapeutischen Konzepts beim malignen Verschlusikterus. Invasive Verfahren wie die Angiographie sind selten indiziert, und einer zunehmenden Zahl von Patienten mit inoperablem Pankreascarcinom kann die ineffektive explorative Laparotomie erspart werden. Die Rate an kurativen Resektionen wird nicht beeinflust.

223. Perioperative Risikoerkennung durch Bestimmung von Faktoren der Blutgerinnung, Fibrinolyse und Wundheilung

Bei 16 Patienten wurden vor und nach grossen Oberbaucheingriffen insgesamt 9 Faktoren der Blutgerinnung, der Fibrinolyse und der Wundheilung bestimmt: Prothrombin, Fibrinogen, Plasminogen, Antithrombin III Alpha-I-Antitrypsin, C-1-Esterase-Inaktivator, Alpha-2-Makroglobulin, Faktor XIII, Fibronectin. Es fand sich zunachst ein signifikanter postoperativer Abfall bei praktisch allen Faktoren. Traten schwere postoperative Komplikationen auf (4 Patienten), so kam es vor allem beim Antithrombin III und Faktor XIII am 3. postoperativen Tag zu einem noch deutlicheren Abfall.
